Traits

Extra Heads. The troll has more than one head. It has advantage on Wisdom (Perception) checks and on saving throws against being blinded, charmed, deafened, frightened, stunned, or knocked unconscious.

Regeneration. The giant regains 30 hit points at the start of its turn. If the giant takes acid or fire damage, this trait doesn’t function at the start of its next turn. The giant dies only if it starts its turn with 0 hit points and doesn’t regenerate.

Vaprak’s Rampage. Upon taking Fire or Acid damage the troll enters a rage. The rage lasts untill the end of its next turn. While raging, the giant gains the following benefits:

  • The giant has advantage on Strength checks and Strength saving throws.
  • When it makes a melee weapon attack, the giant gains a +4 bonus to the damage roll.
  • The giant has resistance to bludgeoning, piercing, and slashing damage.
Actions

Multiattack. The giant makes two Greatclub and uses either a Sling or Swallow attack.

Greatclub. Melee Weapon Attack: +11 to hit, reach 10 ft., one target. Hit: 26 (3d12 + 7) bludgeoning damage and be knocked prone, or 30 (3d12 + 11) bludgeoning damage while raging.

Sling. The troll throws a medium or smaller creature Grappled by it to a space it can see within 60 feet of itself that isn’t in the air. Dexterity Saving Throw: DC 18, the creature thrown and each creature in the destination space. Failure: 26 (4d12) Bludgeoning damage, and the target has the Prone condition. Success: Half damage only.

Swallow. Strength Saving Throw: DC 18, one creature Grappled by the troll (it can have up to 2 creatures swallowed at a time). Failure: 23 (3d8 + 7) Piercing damage. If the target is medium or smaller, it is swallowed and no longer Grappled. A swallowed creature has the Restrained condition, has Total Cover against attacks and other effects outside the troll, and takes 21 (6d6) Acid damage at the start of each of its turns.

If the troll takes 30 damage or more on a single turn from a creature inside it, the troll must succeed on a DC 20 Constitution saving throw at the end of that turn or regurgitate all swallowed creatures, each of which falls in a space within 10 feet of the troll with the Prone condition. If the troll dies, any swallowed creature no longer has the Restrained condition and can escape from the corpse using 15 feet of movement, exiting Prone.

Bonus Actions

Grab. The troll attemps to grab a nearby object or person of medium or smaller stature to Sling or swallow Dexterity Saving Throw: DC 18, one creature Grappled by the troll.

Description

Ancient trolls are far larger and bulkier than their common kin, towering nearly twice a man’s height and thick with corded muscle. Their hide is a mottled gray-green, like stone left to rot, and their claws are as long as daggers. Unlike younger trolls, these creatures slumber for centuries at a time, buried in caves or beneath hills, their bodies half-fused with the earth. When disturbed, they rise in a blind frenzy of hunger and destruction, rampaging until they are grievously harmed. Only then will they crawl back into the dark to heal, their wounds knitting with unnatural speed. Fire and acid remain their only true banes.
