Shapechanger. The were-centaur can use its action to transform between its base centaur form and its hybrid state. In both forms, the curse renders it grotesquely malformed. Its statistics remain the same in each form.

Keen Smell. The were-centaur has advantage on Wisdom (Perception) checks that rely on smell. 

Endless Momentum. The were-centaur doesn’t provoke opportunity attacks when moving in a straight line. If it moves at least 40 feet in a straight line and passes within 5 feet of a creature, that creature must succeed on a DC 19 Dexterity saving throw or take 22(4d10) bludgeoning damage and be knocked prone.

Actions

Multiattack (Hybrid Form Only). The were-centaur makes two attacks: one with its Bite and one with its Hooves.

Bite. Melee Attack Roll: +16, reach 10ft. Hit: 29(4d8 + 10) Piercing damage. 

Hoof Slam. Melee Attack Roll: +16, reach 10ft. Hit: 25(4d6 + 10) Bludgeoning damage. If the target is a Medium or smaller creature and the werecentaur moved 20+ feet straight toward it immediately before the hit, the target takes an extra 27 (6d8) Bludgeoning damage and has the Prone condition.

Recursive Trample. The glitched were-centaur stomps down, causing a warped shockwave to ripple out from its endless form. Each creature within 30 feet of the were-centaur must make a DC 20 Dexterity saving throw, taking 54(12d8) bludgeoning damage on a failed save, or half as much damage on a successful one. A creature that fails the save is also knocked prone.

Bonus Actions

Warped Step. The were-centaur teleports to a space within 30 feet that it can see. This movement ignores terrain and obstacles as it folds through its own glitching mass.

Reactions

Rear Kick (Hybrid Form Only). When a creature enters a space behind the were-centaur within 5 feet, the were-centaur can make a Hooves attack against that creature.

Staggering Echo. When the were-centaur takes damage from a melee attack, it can cause its unstable form to lash out. The attacker must succeed on a DC 18 Wisdom saving throw or have disadvantage on its next attack roll and ability check before the end of its next turn.

Description

A were-centaur is a cursed creature formed when a humanoid is infected with a unique lycanthropy. Instead of becoming a full centaur, the victim grows only the back end of a horse, fused to their lower back. The horse half is scaled to fit the humanoid’s body, with the hind legs matching the human’s leg length. Most were-centaurs are Medium in size, the same as their original form.

When a centaur is infected with this curse, the transformation malfunctions. Their already equine body begins to “glitch,” magically spawning an infinite series of horse hindquarters extending from their back. These distorted forms stretch for miles and miles.

The glitch also affects the mind, making these cursed centaurs far more violent and unstable than regular were-centaurs.
