Traits

Amphibious. The Abyssal leech-fiend can breathe air and water.

Actions

Multiattack. The Abyssal leech-fiend makes up to two arm lash attacks. If either arm is grappling a target then that arm lash attack is replaced by a life leech attack.

Arm Lash. Melee Weapon Attack: +8 to hit, reach 10 ft., one target. Hit: 11 (2d6 + 4) piercing damage plus 3 (1d6) necrotic damage. The target is grappled (escape DC 16) if it is a Large or smaller creature. Until this grapple ends, the Abyssal leech-fiend can't use the same arm on another target.

Life Leech. One target grappled by an arm of the Abyssal leech-fiend must make a DC 14 Constitution saving throw. Failure: The target takes 16 (3d8+3) points of necrotic damage. Success: The target takes half damage. Success or failure: The Abyssal leech-fiend regains hit points equal to half the necrotic damage dealt.

Description

The Abyssal leech-fiend is roughly humanoid in shape, with dark slug-like skin covered in mucus. Its head is joined by a short thick neck and its face has no discernible mouth but is dotted with several small black beady eyes. Its two long flexible snaking arms each end in a horrific flat circular mouth filled with needle-like teeth.

Abyssal leech-fiends are spawned in the deep fetid swamps that lie in several of the Abyssal layers, but many have somehow found their way to the material plane where they lurk in similar environments.

Their preferred method of hunting is to lie in wait, submerged beneath the dark waters until prey passes near enough for a quick strike. But they are also not beyond sneaking into an encampment or nearby village in the dead of night if they think there is a possibility of approaching a sleeping creature undetected. The leech-like mouths on the ends of their arms secrete a pain-numbing chemical: when used on a creature unaware of their presence it is possible they may not even feel it (a DC 14 Perception check - made with disadvantage if the creature is asleep - is required to feel the attack). The fact that a Abyssal leech-fiend was around may not even be known till the following morning and the discovery of withered drained corpses.
