While worn, it empowers the Revenant’s psychic attacks, adding +2 to damage rolls for psychic-based abilities. Spectral Javelins (3) – Ghostly, semi-corporeal weapons the Revenant can conjure and hurl. These vanish after being thrown but regenerate at dawn. Tome of Forgotten Echoes – A ruined book filled with indecipherable glyphs and void-born incantations. While in possession of the tome, the Revenant knows additional void-based spells and can cast Comprehend Languages at will.

Trait Name: Veil of the Endless Night
Description: The Voidborne Revenant is surrounded by a swirling shroud of shadow and cosmic energy. Nonmagical light sources within 30 feet of it dim to a faint glow, and creatures within this radius have disadvantage on Wisdom (Perception) checks that rely on sight. Additionally, the Voidborne Revenant has advantage on saving throws against being blinded or charmed.

Spellcasting. The Voidborne Revenant is an 8th-level spellcaster. Its spellcasting ability is Charisma (spell save DC 17, +9 to hit with spell attacks). The Voidborne Revenant has the following sorcerer spells prepared:

Wailing Void (Recharge 5–6). The Voidborne Revenant releases a terrifying, soul-rending howl in a 40-foot cone. Each creature in that area must make a DC 17 Wisdom saving throw or be stunned and frightened for 1 minute. Affected creatures can repeat the save at the end of each of their turns.

Eldritch Claw. Melee Weapon Attack: +9 to hit, reach 5 ft., one target. Hit: 19 (3d8 + 6) necrotic damage. The target must succeed on a DC 17 Constitution saving throw or have their hit point maximum reduced by the damage dealt until they finish a long rest.

Shadow Javelin. Ranged Weapon Attack: +9 to hit, range 60/240 ft., one target. Hit: 12 (2d6 + 5) piercing damage plus 10 (3d6) psychic damage.

Phantom Strike. Melee Weapon Attack: +11 to hit, reach 5 ft., one target. Hit: 20 (4d6 + 6) psychic damage. The target must succeed on a DC 17 Wisdom saving throw or be stunned until the end of its next turn.

Void Bolt. Ranged Weapon Attack: +10 to hit, range 90/300 ft., one target. Hit: 15 (3d6 + 6) necrotic damage plus 7 (2d6) force damage.

Ethereal Slip. The Voidborne Revenant moves up to 30 feet without provoking opportunity attacks. It can move through creatures and objects as if they were difficult terrain until the start of its next turn.

Ethereal Shift
Description: The Voidborne Revenant blurs into the Ethereal Plane. Until the start of its next turn, it has resistance to all damage except force and radiant, and it can move through creatures and objects as if they were difficult terrain. If it ends its turn inside an object, it is shunted to the nearest unoccupied space and takes 1d10 force damage.


Voidstep
Description: The Voidborne Revenant teleports up to 30 feet to an unoccupied space it can see. Creatures within 5 feet of the space it left must succeed on a DC 17 Wisdom saving throw or be frightened until the end of their next turn.

Reactions

Shadow Counter
Description: When a creature misses the Voidborne Revenant with a melee attack, it can use its reaction to lash out with a shadowy tendril. The attacker must succeed on a DC 17 Wisdom saving throw or take 12 (3d8) psychic damage.


Ethereal Parry
Description: When hit by a melee attack, the Voidborne Revenant can partially shift into the Ethereal Plane, imposing disadvantage on the attacker’s next attack roll before the end of the Voidborne Revenant’s next turn.


Mind Spike
Description: When a creature within 30 feet casts a spell, the Voidborne Revenant can use its reaction to force the caster to make a DC 17 Intelligence saving throw or take 14 (4d6) psychic damage.

Legendary Actions

Legendary Actions.
The Voidborne Revenant can take 6 legendary actions, choosing from the options below. Only one legendary action can be used at a time and only at the end of another creature's turn. The Voidborne Revenant regains spent legendary actions at the start of its turn.


Whisper in the Dark. The Voidborne Revenant targets one creature it can see within 60 feet. That creature must succeed on a DC 17 Wisdom saving throw or hear maddening whispers, giving it disadvantages on its next attack roll or saving throw (whichever comes first).


Phase Lurch. The Voidborne Revenant teleports up to 20 feet to an unoccupied space it can see. This movement does not provoke opportunity attacks.


Fracture Reality (Costs 2 Actions). The Revenant warps the space around a point it can see within 60 feet. Each creature in a 10-foot radius must succeed on a DC 17 Constitution saving throw or take 13 (3d8) force damage and be knocked prone.


Voidlash. The Voidborne Revenant makes one Eldritch Claw or Phantom Strike melee attack.


Mind Rupture (Costs 2 Actions). The Revenant targets one creature it can see within 30 feet. That creature must succeed on a DC 17 Intelligence saving throw or take 18 (4d8) psychic damage and be unable to take reactions until the end of its next turn.


Shatter Thoughtform (Costs 3 Actions). The Revenant emits a pulse of void energy. All creatures within 30 feet must make a DC 17 Wisdom saving throw. On a failed save, a creature is stunned until the end of its next turn. A creature that succeeds is immune to this effect until the end of the Revenant’s next turn.

Description

The Voidborne Revenant is a spectral abomination born from the collapse of a mortal soul into the void between worlds. Once a powerful spellcaster who delved too deep into forbidden cosmic knowledge, it was consumed by the very powers it sought to command. Now, its form flickers with shadow and stars, like a living tear in reality. Whispering voices swirl in its presence, and its eyes glow with a cold, unnatural light.

Driven by a hunger for minds and memories, the Voidborne Revenant is drawn to arcane energy and intelligent prey. It speaks little, preferring to communicate telepathically in cryptic riddles or fragments of forgotten language. The air around it warps subtly—shadows lengthen, light dims, and colors dull, creating an oppressive sense of unreality.

In battle, it drifts above the ground with ethereal grace, striking with piercing force and mental anguish. It does not fight for territory or vengeance—it fights to silence the noise of existence, to drag others into the stillness of the void that created it.
