Psionic Runes. Infernal glyphs writhe across the Velthaar’s flesh. These runes render it immune to the domination of Elder Brains and other illithids.
Magic Resistance. The Velthaar has advantage on saving throws against spells and other magical effects.
Innate Psionics. The Velthaar’s spellcasting ability is Intelligence (spell save DC 15). It can innately cast the following spells, requiring no material components:

Tentacles. Melee Weapon Attack: +6 to hit, reach 10 ft., one target. Hit: 14 (2d10 + 3) psychic damage, and the target is grappled (escape DC 14). Until this grapple ends, the Velthaar can’t use its tentacles on another target.
Psionics. The Velthaar casts one of its known spells.
Extract Brain. Melee Weapon Attack: +6 to hit, reach 5 ft., one incapacitated humanoid grappled by the Velthaar. Hit: 55 (10d10) piercing damage. If this damage reduces the target to 0 hit points, the Velthaar kills the target by extracting and devouring its brain.
Whip-Tail. Melee Weapon Attack: +6 to hit, reach 15 ft., one target. Hit: 11 (2d6 + 3) slashing damage + 7 (2d6) fire damage.
Immolation Wave (Recharge 5–6).
The Velthaar unleashes a psionically-charged eruption of flame and thought in a 60-foot cone.

Effect: Each creature in the area must make a DC 15 Intelligence saving throw.

On a failed save: A creature takes 20(5d8) fire damage and 20(4d8) psychic damage, and suffers Burned Synapses.

On a successful save: A creature takes half damage and does not gain Burned Synapses.
*Burned Synapses. A creature afflicted takes 1d4 fire and 1d4 psychic damage at the start of each of its turns. The effect ends after 3 rounds, or if the creature uses its action to douse itself.
Hellish Step (1/Combat).
The Velthaar teleports up to 60 feet to an unoccupied space it can see. When it appears, the ground within 10 feet of its destination erupts in elemental flame. All creatures in that area automatically take 5 (1d8) fire damage — no save.
Hellish Rebuke(3/day)
Once Per round, in response to taking damage, the Velthaar can cast Hellish Rebuke as a reaction onto their attacker
Description
Born only when a Tiefling is subjected to ceremorphosis with a Helltouched Tadpole — a mind flayer larva infused with Infernal Flame by a denizen of Baator — a Velthaar is a grotesque fusion of psionic corruption and infernal legacy. Its body is lean and spined, colored a mottled dark mauve. Jagged horns split and writhe like living bone, framing a face where smoke curls with every labored breath. But truly, no Velthaar's horns look the same beyond those characteristics. Some may have a crown of horns, others may have tendrils where horns once were. Its eyes are hollow sockets lit by pinpricks of burning coals, and its tail has unraveled into a cluster of whip-like tendrils.
Velthaar carry the unnerving sweetness of the illithid’s vanilla-scented slime, tainted by acrid sulfur. Their presence is heralded by a heat-haze shimmer and the faint scent of brimstone. Telepathic speech echoes as two voices layered together: one psionic whisper, one infernal growl.
Rogue Aberrations
The infernal runes seared across a Velthaar’s flesh writhe like living scars, pulsing in time with its psychic power. These glyphs shield it from the influence of any Elder Brain, rendering it a rogue aberration hated by its kin. Illithid colonies see Velthaar as dangerous mistakes, and most hunt them relentlessly. In contrast, Baatorian cults and infernal warlords often prize Velthaar as proof that even the psionic can be bound to Hell’s design.
Pyro-Psionic Horrors
Velthaar revel in destruction, igniting minds and flesh alike. Their psionics manifest as living flame, blasting thoughts into firestorms that linger as Burned Synapses, searing both body and sanity. They move with infernal theatrics, vanishing in a blaze of Hellish Step to appear amid their prey, fire erupting around them as reality cracks.
*Author's Note: Helltouched Tadpoles are often created when a Devil makes a contract with an Illithid. Much like how Tieflings often come from humanoids who make contracts with devils, Helltouched Tadpoles are spawned by Illithids who were foolish enough to cavort with the Infernal, as Illithid naturally lay eggs at several stages of their life. Helltouched Tadpoles are darker, spinier, and most implantations into non-tieflings result in half burned illithid rejections.
