Traits

Tentacle. A limb which is part of a much larger creature, this entity can move up to 100ft away from the main body of the creature, the death of this limb is not permanent so long as the main body survives.

Spread Out : The main center of mass (the "hitbox") of a Tentacle will never be within 20 ft of another tentacle and will attempt to enter a strategic positioning based upon what attacks they will use. At least two tentacles MUST be on either side of the Watching Stone so as to maintain its balance. Tentacles not on the same side of the Watching Stone as its targets are unable to use their "Sweep" or "Slam" attacks.

Actions

Grab. The tentacle will grab its target in an attempt to grapple them. If successful, it will deal 2D10 piercing damage per turn the target is grappled, and attempt to bring the target to the main body to be consumed. While grappling a target, its movement speed is reduced to 30.

Sweep. Melee Weapon Attack:  reach 60 ft within 100ft of the Watching Stone., 80 ft cone that does not go beyond 100 ft of the Watching Stone. Hit:  5D8+10 magical slashing damage and an additional 3D10 magical bludgeoning damage. Ignores immunities. Targets must make a DC 24 Dexterity save or take full damage. A successful save removes the bludgeoning damage entirely.

Slam: Melee Weapon Attack. The Tentacle attempts to crush its enemies with a massive slam against the ground. Reach: 100ft from the Watching Stone. 20 ft wide line from the Watching stone. Hit: 5D10+10 magical bludgeoning damage, and an addition 3D10 magical piercing damage. Ignores Immunities. Targets must make a DC 24 Dexterity save or take full damage. A successful save removes the Piercing damage entirely.

Javelin. Natural ranged attack:  The tentacle will launch itself at a target that is within 100 ft of the Wandering Stone. +20 to hit, 1 target. Hit:  5D12+15 Magical Piercing Damage. Ignores Immunities. Only 2 tentacles can use this attack per turn of The Watching Stone. 

Boulder Throw. Natural Ranged Attack. The Tentacle will throw a large rock at a target that is beyond its reach. Reach: 300 ft. 20ft AOE. +15 to hit. 1 target. Hit: 3D10+20 Bludgeoning Damage. Ignores immunities. Airborne targets must make a DC 25 constitution saving throw or fall to the ground. Targets must make a DC 22 Dexterity saving throw to halve the damage.

Description

One of eight arms of an Ancient Watching Stone. Protected by a slime on its thick skin and armed with vast amounts of hooks along them, tipped in a spear like claw. These mobile limbs can bring down entire groups of the unprepared.
