Eldritch Presence. Any creature that starts its turn within 30 feet of the Visionscourge must succeed on a DC 15 Wisdom saving throw or see fleeting, surreal visions that impose disadvantage on its next attack roll.
Unstable Form. The Visionscourge appears partially real, shifting between forms. Attacks against it have disadvantage unless the attacker has truesight.
Multiattack. The Visionscourge makes two Dreamlance attacks.
Dreamlance (Ranged Spell Attack). +7 to hit, range 90 ft., one target. Hit: 21 (6d6) psychic damage. The target must succeed on a DC 15 Intelligence saving throw or be Stunned until the end of its next turn, overwhelmed by alien visions of impossible worlds.
Mindrend Gaze (Recharge 5–6). The Visionscourge locks eyes with up to 3 creatures of its choice within 60 feet. Each must make a DC 15 Intelligence saving throw. On a failed save, the creature takes 27 (6d8) psychic damage and is incapacitated until the start of its next turn, trapped in a flood of nightmare imagery. On a success, it takes half damage and suffers no other effects.
The Visionscourge can take 2 legendary actions, choosing from the options below. Only one legendary action can be used at a time, and only at the end of another creature’s turn.

Whispering Echo. The Visionscourge targets one creature it can see within 30 ft. That creature must succeed on a DC 15 Wisdom saving throw or hear whispers that impose disadvantage on its next saving throw.

Shift Form. The Visionscourge moves up to half its speed without provoking opportunity attacks.
