Shapechanger: The werefrog can use its action to polymorph into a Large frog-humanoid hybrid or a Large giant frog form, or back into its humanoid form. 

  • In frog form, its size becomes Large, AC becomes 13, and its bite deals +1d4 extra damage.
  • Equipment does not transform; the werefrog's clothes or tear as appropriate.

Standing Leap: The werefrog's long jump is up to 30 feet and it's high jump is up to 15 feet, with or without a running start.

Amphibious: The werefrog can breathe air and water.

Actions

Multiattack: The werefrog makes two attacks, only one of which can be a bite

Bite (Frog or Hybrid Form Only). Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 14 (2d8 + 4) piercing damage. If the target is a humanoid, it must succeed on a DC 11 Constitution saving throw or be cursed with werefrog lycanthropy. They must also attempt a DC 15 Strength saving throw or be grappled (escape DC 15).

Claws (hybrid Form Only):  Melee Weapon Attack: +7 to hit, reach 5ft., one target. Hit: 11 (2d6+4) slashing damage.

Tongue (Hybrid or Frog Form):  Ranged Weapon Attack: +7 to hit, range 15/30 ft., one target. Hit: 8 (1d6 + 4) bludgeoning damage, and the target is pulled up to 15 feet toward the werefrog. If the target ends this movement adjacent to the werefrog, it is grappled (escape DC 15).

Swallow (Frog Form Only): The werefrog makes a Bite attack against a Small or smaller grappled creature. If it hits, the target is swallowed.

  • The swallowed creature is blinded and restrained, has total cover, and takes 10 (3d6) acid damage at the start of each of the werefrog's turns.
  • If the werefrog takes more than 25 or more damage on a single turn from the swallowed target, the werefrog must take a DC 15 Constitution save or regurgitate the creature.
Reactions

Sudden Leap: When a creature ends its turn within 10 feet of te werefrog, it may jump up to 15 feet without provoking opputunity attacks (unless below 1/4 health).

Legendary Actions

The werefrog can take 2 legendary actions, using the following:

Leap: The werefrog jumps up to 15 feet.

Tongue Lash: Make a Tongue attack.
