Mineral Plating
The Unholy Amalgamation of Flesh and Stone’s (UAFS) body is encased in stone-like armor. It has resistance to nonmagical bludgeoning, piercing, and slashing damage, and has advantage on saving throws against being pushed, knocked prone, or grabbed.

Earth-Anchor Stability
If the UAFS starts its turn touching the ground, it gains 10 temporary hit points.  It loses these if lifted into the air.

Residual Memory
When reduced to half its maximum HP for the first time, the UAFS emits fractured memories in broken speech.  Any PC who has fought the creature in any of its previous forms recognizes it as fragments of its previous form(s).

Actions

Multiattack.
The UAFS makes two attacks:  one with its Slam attack and one with its Rend attack, or two Slam attacks.

Slam. Melee Weapon Attack: +8 to hit, reach 10 ft., one target. Hit: 18 (2d8 + 5) bludgeoning damage.

Rend. Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit: 18 (3d8 + 5) slashing damage.

Elemental Howl. (Recharge 5-6) 
The UAFS lets out a grinding, tearing howl of earth and flesh.  Each creature within 20 feet must make a DC14 Wisdom save or take 18 (4d8) psychic damage and become Frightened until the end of its next turn.

Earth Anchor Charge. (Recharge 4-6) 
The UAFS surges forward in a straight line up to 20 ft.  Each creature in the path must make a DC 15 Strength save or take 22 (4d10) bludgeoning damage and be knocked prone.  On a success, the target takes half damage and is not knocked prone.  If it ends the movement adjacent to a creature, it may make one Slam attack as a bonus action.

Legendary Actions

The UAFS has 2 Legendary Actions per round.

Move (Costs 1). The amalgamation moves up to half its speed.

Slam (Costs 2). Makes one Slam attack.

Description

A nightmare of warped anatomy and elemental brutality, this creature towers nearly twice the height of an ogre, its grotesque frame stitched together from masses of swollen flesh and jagged, slate-gray stone. Thick plates of rock bulge from its shoulders, arms, and spine like a natural suit of armor, shot through with glowing crimson fissures that pulse like veins filled with magma. Its face is a malformed parody of life—half skull, half crag—bearing too many teeth and eyes that burn like molten coals. Each movement grinds stone against stone, and each breath whistles through vents of cracked rock as if the beast itself were a living cavern. When its claws scrape the floor, you see that even they are fused with broken bits of masonry, torn from whatever depths birthed this abomination.
