Traits

Lethal Gaze. If a creature starts its turn within 20 feet of the Ancient Basilisk and the two of them can see each other, the Ancient Basilisk can force the creature to make a DC 14 Constitution saving throw if the Ancient Basilisk isn't incapacitated. On a failed save, the creature takes 10 (3d6) necrotic damage, has their armor class reduced by 3 and magically begins to turn to stone and is restrained. It must repeat the saving throw at the end of its next turn. On a success, the creature is no longer restrained but their armor class remains reduced for 1 hour. On a failure, the creature is petrified for 7 days or until freed by the greater restoration spell or other magic. A creature who succeeds on this saving throw has advantage on all saving throws against the Lethal Gaze of all Ancient Basilisks for the next hour.

A creature that isn't surprised can avert its eyes to avoid the saving throw at the start of its turn. If it does so, it can't see the Ancient Basilisk until the start of its next turn, when it can avert its eyes again.

If it looks at the Ancient Basilisk in the meantime, it must immediately make the save. If the Ancient Basilisk sees its reflection within 30 feet of it in bright light, it mistakes itself for a rival and targets itself with its gaze.

Regeneration. The Ancient Basilisk regains 24 hit points at the start of its turn if it has at least 1 hit point remaining. If the Ancient Basilisk takes acid or fire damage, this trait doesn't function at the start of the Ancient Basilisk's next turn.

Actions

Multiattack. The Ancient Basilisk makes two bite attacks each of which must have a different target.

Bite. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 19 (2d10 + 3) piercing damage plus 6 (1d10) poison damage.
