Dual Aspect.
The Avatar exists as both creation and finality.
At the start of its turn, it chooses one of the following aspects until the start of its next turn:
Aspect of Io (Possibility Unbound)

Regains 15 hit points.

Spell attacks have advantage.

Once per turn, when it deals damage, it may reroll any number of damage dice and use the new results.
Aspect of Ao (Final Law)

Gains resistance to all damage until the start of its next turn.

Creatures of its choice within 20 feet have disadvantage on saving throws.

Once per turn, when it hits, it can force a Con save (DC 19) or the target is restrained by crystallized causality until the end of their next turn.
Legendary Resistance (3/day). If the Avatar fails a saving throw, it can choose to succeed instead.
Immutable Verdict. The Avatar is immune to any effect that would alter its form, erase it from existence, rewrite its history, or trap it outside reality (such as maze, banishment, or imprisonment). Such effects automatically fail.
Warlock Spellcasting. The Avatar is a 13th-level warlock-equivalent spellcaster. Its spellcasting ability is Charisma (spell save DC 19, +11 to hit with spell attacks).
It does not use spell slots—it can cast each spell below at will unless noted otherwise.

Multiattack. The Avatar makes two Verdict of Being attacks or casts a spell and makes one attack.
Verdict of Being. Melee or Ranged Spell Attack: +11 to hit, reach 10 ft., one target. Hit: 22 (4d8 + 4) force damage.
If in Aspect of Ao, the target must succeed on a DC 19 Wisdom save or be Marked for Conclusion until the end of its next turn. A marked creature has disadvantage on all attack rolls and saving throws.
If in Aspect of Io, the Avatar may immediately teleport to an unoccupied space within 30 feet after this attack.
Collapse Possibility (Recharge 5–6). Reality compresses into a single correct outcome.
All creatures of the Avatar’s choice within a 30-foot radius must make a DC 19 Charisma saving throw.
On a failure:

On a success:

Unwrite Anomaly(1/day): The Avatar targets one creature it can see within 60 feet.
The target must make a DC 19 Charisma saving throw.
On a failure, the creature is reduced to 0 hit points and becomes a fading silhouette of unreal memory. If it dies while in this state, it cannot be revived except by wish or divine intervention.
On a success, the creature takes 55 (10d10) psychic and force damage and is frightened until the end of its next turn.
Retroactive Denial. When a creature within 60 feet succeeds on an attack roll or saving throw, the Avatar may force it to reroll and take the lower result.
The Avatar can take 3 legendary actions, choosing from the options below.
Eldritch Pulse: Casts eldritch blast.
Warp Continuity: Teleports up to 40 feet and becomes incorporeal until the end of its next turn.
Final Word (Costs 2 Actions): One creature within 60 feet must succeed on a DC 19 Wisdom save or be silenced, slowed, and unable to regain hit points until the end of its next turn.
Rewrite Outcome (Costs 3 Actions): The Avatar forces a creature to reroll any d20 roll it just made.
Mythic Phase. When the Avatar is reduced to 0 hit points, it does not die.
Instead, reality stutters.
It regains 150 hit points, and both aspects fully manifest.

Lore & Manifestation
The Avatar of Ao–Io does not arrive with thunder or fire. It concludes into being.
Reality tightens around it like a held breath. Possibility bleeds from its left side—fractured constellations, half-formed wings, overlapping faces, unborn stars flickering beneath translucent skin. This is Io, the endless yes, the instinct to make without asking why.
Its right side is immaculate, geometric, inevitable—smooth stone, perfect angles, eyes like collapsing horizons. This is Ao, the final word, the law that remembers every cost.
It speaks rarely. When it does, its voice overlaps itself—one tone asking what could be, the other declaring what must be.
It is not cruel.
It is not kind.
It is necessary.
Where it walks, paradox stabilizes. Immortals feel old. Time remembers itself. Spells hesitate.
This avatar is dispatched only when reality itself cannot decide what it is anymore.
Lair and Lair Actions
The Avatar’s lair is not a place so much as a decision point in existence—a ruined god-forge, a shattered timeline knot, a world-heart, or a paradox scar. Within it, stories collapse into truths.
On initiative count 20, the Avatar may use one of the following lair actions:
Lair Actions: The Verdict Nexus
1. Paradox Bloom (Io Aspect Favor): Impossible terrain erupts.
Choose up to three 10-foot cubes within 120 feet. Each becomes warped by unreal geometry until the next round:

The area is difficult terrain.

Creatures that enter or start their turn there must succeed on a DC 19 Wisdom save or be disoriented, granting advantage on the next attack against them.

Illusions, spectral wings, half-formed staircases, or floating fragments appear.
If the Avatar is in Aspect of Io, creatures that fail the save also take 14 (4d6) radiant or psychic damage (Avatar’s choice).
2. Law of Final Shape (Ao Aspect Favor): Ao asserts the correct version of reality.
Choose a point within 120 feet. All creatures in a 20-foot radius sphere must make a DC 19 Constitution save.
On a failure:

Speed becomes 0 until initiative count 20 next round

Reactions are disabled

They cannot teleport or become incorporeal
On a success:

Speed is halved

No reactions
If the Avatar is in Aspect of Ao, creatures who fail also take 18 (4d8) force damage.
3. Collapse Possibility: All advantage and disadvantage within the lair are erased until the next round.
Additionally, the Avatar chooses one creature it can see. That creature must reroll its next d20 roll and take the lower result.
This is not magical—it is existential.
4. Fracture of Timelines: Echoes of what could have been intrude.
Up to three shadowy duplicates of creatures that have taken turns since the last lair action appear in unoccupied spaces near them. Each duplicate:

Has AC 15, 1 HP

Mimics one weapon or cantrip attack immediately

Then shatters
If a duplicate hits, it deals 10 (3d6) force damage.
5. Unstable Verdict (Recharge 6): The lair itself speaks judgment.
Every creature of the Avatar’s choice must make a DC 19 Charisma saving throw.
On a failure, the creature is affected by one of the following (roll 1d4 or choose):

Unwritten: The creature becomes invisible and intangible until the end of its next turn (cannot act).

Overwritten: The creature is restrained and takes 22 (5d8) psychic damage.

Corrected: The creature is forcibly teleported to a location the Avatar chooses within 60 feet.

Revised: The creature’s last action is retroactively undone (DM adjudicates).
