Ethereal: The Avatar exists only on the ethereal plane, it can't affect or be affected by anything on the material plane, other than the target of Chaos Surge.
Tethered: The Avatar must remain within 60 ft. of the target at all times.
Multiattack: The Avatar makes three attacks with its Elemental Rapier, or replacing one rapier attack with Wrathful Shreak
Elemental Rapier: Melee Weapon Attack: +10 to hit, reach 5 ft., one target. Hit: 8 (1d8 + 4) piercing damage, plus 27 (6d8) acid, cold, fire, lightning, or poison damage.
Wrathful Shreak: The Avatar lets out a piercing cry. Each creature in a 40 ft. cone must make a DC 18 Wisdom saving throw, taking 25 (4d12) psychic damage and being paralyzed until the end of the Avatar's next turn on a fail, are taking half damage on a success.
Chaos Surge(2/long rest): The Avatar chooses either a boon or bane for their target, but not the effect, this action is always accompanied by disembodied chanting that only the target can hear. These effects last until Chaos Surge is used again, unless specified as ongoing(*).

6 The target's mind becomes telepathically linked to an ally's, they can tell the exact location of one another, as well

14 Gains the ability to make two petrifying attacks. DC 13 Constitution saving throw, an a fail, the target is restrained. At the end of the target's next turn, it must make another save, on a fail, the target is petrified.
15 Target can walk on water for 10 minutes after saying a command word, this works 1d4 times
16 The target's tongue becomes sleek and forked, they gain blindsight out to 10 ft., as long as they continuously flick their tongue
17 Gain 1 9th level spell scroll of the DM's choosing(*)
18 1 of the target's magic items becomes sentient(*)
19 Gains an animal companion of CR 1 or lower(*)

1 A nonharmful Symbol effect is cast at the target every 2d6 minutes
2 Target takes on the appearance of a hybrid formed lycanthrope, but otherwise is unchanged
3 Target grows donkey ears and a tail, every time they take an action in combat, roll a d20, on a 1, they immediately fail their action, instead loudly braying for their whole turn
4 Target sheds dim blue light out to 10 ft., and has disadvantage on stealth
5 Gains vulnerability to a damage type
6 Target grows (Enlarge/Reduce)

8 The target swaps minds with another person

Description
Ancient rituals can tie these rare and unpredictable entities to a mortal creature, most times unwillingly. Avatars of Chaos were once spirits of the Feywild, though through strong binding magic, they are now tethered to one creature, and are forced to take all aggression out on them. These beings look like fey versions of their target. The Avatar is freed either when their target dies, or if a Banishment spell is cast on the target.
The avatars forced to torment unaware mortals often try to find mischief in the face of their own misfortune, but even an angry avatar may grow sympathetic. One of these spirits my give the occasional boon to a downtrodden target, though these favors vary in frequency, if at all.
These rituals were once used by archfey to defend their homeland, though mortals have since misused it for their own gain, and the suffering of their foes.
