Heart of the Elder Evils. the [REDACTED] can mimic any sound it has heard, but it tends to stay in the shadows to avoid being exposed to escapees.

Gatekeeper of the Truth. the [REDACTED] deals more damage depending on how much they know about life, and the [REDACTED] can tell if a creature is lying or not.

Havoc. the [REDACTED] can choose 1 main class feature (e.g. Rage for Barbarian, Divine Intervention for Cleric, etc) to be used for it's turn in combat.

Magic Absorbant. the [REDACTED] returns all magic to the senders (see MYTHIC ACTION) and is also immune to spell slots above class level 10.

Weaponry Hands. the [REDACTED] can create a weapon out of thin air, despite gaining all the bonuses a normal weapon would have, it's still a Rend action

Mythic Actions

the [REDACTED] can take the following Mythic Actions, choosing from the options below, the Mythic Actions only apply to the creatures that damaged it, the [REDACTED] can only take one Mythic Action at a time and at the start of it's turn if it has taken damage, the [REDACTED] regains all expended uses at the start of it's next turn.

Heal. the [REDACTED] heals HP equal to the amount it has taken up to its last turn, regardless of which type of damage.

Counter. the [REDACTED] launches a wave of whichever element it chooses, the creatures in it's way must make a DEX save or take 74 (15d4 + 30) [damage type] damage on a failed save, or half as much on a successful one.

Charge up (Costs 3 Actions). the [REDACTED] takes 18s to wind up an attack, brace for impact, when the attack launches, roll a CON save or take all the magical damage that struck it in one massive, reality destroying beam of pure void, a success means that the creature is superpowered by the beam.

Description

Log 1423: the Arrival.

I saw this thing the first time, it felt like it was a normal T-rex, but it was so, so much more unsolved, it didn't have the usual green scales, gigantic posture, or loud roars, this one was purple, the size of a Lion, and made the same noises as a pseudodragon, I went closer and, that's when it showed it's true self.

Log 1424: the Research.

I can't put into words on how to describe it, because I'm afraid that it can hear me from the core of the earth, as it headed into the earth, I thought it was the end, until I saw it massacre a man in plain sight out of nowhere, "this thing is vicious," I said.

Log 1425: the Hunt.

I later dug and found out it doesn't just target random creatures, it targets those who seek to know everything, including itself, so I had to act fast so it doesn't get me, I barricaded doors and windows, locked my lab, and stayed in my bedroom on the top floor of my house, then it came from under the bed, staring at me with it's now grotesque looks in my face, if I could describe this monster, I was as good as dead.

Log 1426: the Survival.

I am now in my bunker way above the clouds, I know it can spontaneously spawn in earth anywhere there is research, so I did as much of it as I could, I later realized it was near immortal, no one has ever killed it, and it is prophesized that it is a gatekeeper of every secret there is.

Log 1427: the Conclusion.

I later gave it the specimen name of "Specimen 3650: SECROTAR" because it's not an experiment, it's a lost God, the last known bit of info I can think of is that it's an Elder Evil, and it wanted me, Dr Leo Snow, good as dead, I figured out it's strategy in combat to find out what it does and documented all of it on a scroll, I will describe what it looks like when it's more dead than a lich, end of log.
