Aura of Dread. Whenever a creature starts their turn within 20 feet of the dragon, they must succeed on a DC 21 Wisdom saving throw or be frightened until the start of their next turn.

Legendary Resistance (3/Day). If the dragon fails a saving throw, it can choose to succeed instead.

Actions

Multiattack. The dragon can use its Frightful Presence. It then makes one bite attack and two claw attacks.

Bite. Melee Weapon Attack: +15 to hit, reach 10 ft., one target. Hit: 20 (2d10 + 9) piercing damage plus 3 (1d6) psychic damage. If the target was a creature, it must succeed on a DC 20 Wisdom saving throw or be frightened until the start of its next turn.

Claw. Melee Weapon Attack: +15 to hit, reach 5 ft., one target. Hit: 16 (2d6 + 9) slashing damage.

Tail. Melee Weapon Attack: +15 to hit, reach 20 ft., one target. Hit: 18 (2d8 + 9) bludgeoning damage, and the target is knocked prone.

Frightful Presence. Each creature of the dragon’s choice that is within 120 feet of the dragon and aware of it must succeed on a DC 21 Wisdom saving throw or become frightened for 1 minute. A creature can repeat the saving throw at the end of each of its turns, with disadvantage if it can see the dragon, ending the effect on itself on a success. If a creature’s saving throw is successful or the effect ends for it, the creature is immune to the dragon’s Frightful Presence for the next 24 hours.

Fear Breath (Recharge 5-6). The dragon exhales an invisible ray of fear in a 90-foot line that is 5 feet wide. Each creature in that area must make a DC 21 Wisdom saving throw, taking 66 (12d10) psychic damage and be frightened for 1 minute on a failed save, or half as much damage only on a successful one. A frightened creature can repeat the saving throw on the end of each of it's turns, ending the effect on a success.

Bonus Actions

Terrorizing Restoration (Recharge 5-6). The dragon magically heals itself using the fear of other creatures. The dragon gains 4 (1d8) hit points for each frightened creature within 40 feet of it.

Legendary Actions

The dragon can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The dragon regains spent legendary actions at the start of its turn.

Detect. The dragon makes a Wisdom (Perception) check.

Tail Attack. The dragon makes a tail attack.

Wing Attack (Costs 2 Actions). The dragon beats its wings. Each creature within 10 feet of the dragon must succeed on a DC 23 Dexterity saving throw or take 16 (2d6 + 9) bludgeoning damage and be knocked prone. The dragon can then fly up to half its flying speed.

Frightening Roar (Costs 3 Actions). The dragon frighteningly roars, which is audible up to a mile away. Each creature within 600 feet of the dragon must succeed a DC 21 Wisdom check or be frightened of the dragon for 1 minute. Creatures more than 100 feet away have advantage on this saving throw. At the end of a frightened creature's turn, they can make the saving throw again, ending the effect on a success.

Fully grown, it stands as a towering, terrible silhouette of pale scales and shadowed edges, a being whose presence seems to drain color from the world. Its matte white hide has hardened into interlocking plates that resemble carved bone, each one etched with faint, branching lines like stress fractures in reality itself.

The black spines that once lined its back have become long, jagged structures of obsidian‑like material, each one humming with psychic resonance. When the dragon is agitated, these spines vibrate in discordant harmony, producing a low, thrumming sound that worms its way into the minds of all nearby. Even the bravest warriors feel their resolve falter under that sound, as if their courage is being peeled away layer by layer.

Its wings are vast, cathedral‑like structures of pale membrane stretched over darkened bone. When unfurled, they cast shadows that seem to move independently of the dragon’s body, flickering with shapes that resemble half‑remembered nightmares. In flight, the dragon does not roar — it silences the world beneath it, leaving only the sound of one’s own heartbeat pounding in their ears.

Lair and Lair Actions

The lair is a predatory mindscape made physical, a place where fear behaves like weather. Shadows cling too long to corners, sounds arrive a heartbeat late, and the air itself feels aware—watching, waiting, learning what terrifies intruders most.

The walls are carved smooth by centuries of psychic pressure, etched with faint grooves that shift when not observed. Echoes drift without sources, repeating fragments of the party’s own voices in tones that don’t quite match. Pools of darkness gather in low places, swallowing light rather than reflecting it.

Every step deeper into the lair tightens the atmosphere. The temperature doesn’t drop, but the sense of being hunted rises sharply. Hallucinations flicker at the edge of vision—never fully formed, never repeated, always tailored. The lair reacts like a living organism, adjusting its layout subtly to isolate intruders, distort distances, and funnel them toward the dragon’s chosen ground.

At the heart of the lair lies a cavern where the stone is polished mirror‑smooth, reflecting not the intruder’s body but their fear‑warped silhouette. This is where the Fear Dragon waits, perfectly still, perfectly silent, letting the lair soften its prey before it strikes.

 An adult fear dragon in its lair has a challenge rating of 20 (25,000 XP).

Lair Actions

On initiative count 20 (losing initiative ties), the fear dragon takes a lair action to cause one of the following effects; the fear dragon can’t use the same effect two rounds in a row:

  • Hallucinatory gas seeps through cracks in the walls in a 40-foot radius centered on a point within 120 feet of the dragon that it can see. Each creature in that radius must succeed on a DC 21 Wisdom saving throw or be frightened until the end of the dragon's next turn. While frightened this way, the target is incapacitated.
  • The fear dragon targets one frightened creature within 10 feet of itself. That creature must succeed on a DC 21 Wisdom saving throw or lose the frightened condition and be charmed for the same duration instead. While charmed this way, the dragon controls the creature, and the creature's stat block is replaced by that of a shadow. The charmed creature can make the saving throw again on the end of its turn, ending the effect on a success.
  • A surge of psychic dread erupts from a point the dragon chooses within 120 feet. Each creature within a 30‑foot radius of that point must succeed on a DC 21 Wisdom saving throw or be paralyzed.

Regional Effects

The region containing a legendary fear dragon’s lair is warped by the dragon's magic, which creates one or more of the following effects:

  • Whenever a creature is within 6 miles of the lair and takes a long rest, it must make a DC 21 Wisdom saving throw. On a failure, the creature gains no benefit from the long rest.
  • Creatures that enter the lair with short-term or long-term madness move one step up in madness (short-term to long-term, long-term to indefinite).
  • The fear dragon's essence creates numerous rifts to the Shadowfell. Creatures from the Shadowfell can use an action to go through the rift to the other end, given they are 5 feet away from it and are not incapacitated. A creature that enters the rift is teleported to an unoccupied space within 5 feet of the rift. Once a creature passes through, the rift collapses.

If the dragon dies, these effects fade over the course of 3d10 days.
