Hold Breath. Thousand Teeth can hold its breath for 30 minutes.

Swamp Camouflage. While in swamp terrain or murky water, Thousand Teeth has advantage on Dexterity (Stealth) checks and can attempt to hide as a bonus action.

Legendary Resistance (2/Day). If Thousand Teeth fails a saving throw, it can choose to succeed instead.

Blood Frenzy. Thousand Teeth has advantage on melee attack rolls against any creature that is below half of their total hit points.

Actions

Multiattack. Thousand Teeth makes two attacks: one with its bite and one with its tail.

Bite. Melee Weapon Attack: +10 to hit, reach 10 ft., one target. Hit: 20 (3d8 + 6) piercing damage. If the target is Large or smaller, it is grappled (escape DC 16 Strength saving Throw). Until this grapple ends, the target is restrained, and Thousand Teeth can't bite another target.

Tail. Melee Weapon Attack: +10 to hit, reach 10 ft., one target. Hit: 15 (2d8 + 6) bludgeoning damage. If the target is a creature, it must succeed on a DC 16 Dexterity saving throw or be knocked prone.

Death Roll (Recharge 5-6). Thousand Teeth makes a bite against a creature it is grappling. ON a hit, the target takes an extra 10 (3d6 bludgeoning damage and must succeed on a DC 16 Strength saving throw or be knocked prone and dragged up to 15 ft. If the creature is dragged into water, it is submerged.

Legendary Actions

Thousand Teeth can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature’s turn. Thousand Teeth regains spent legendary actions at the start of its turn.

Detect. Thousand Teeth makes a Wisdom (Perception) check.

Thrash. Creatures within 5 feet must succeed on a DC 16 dexterity saving throw or fall prone.

Lunge. Thousand Teeth moves up to half its speed without provoking opportunity attacks.

Bite (Costs 2 Actions). Thousand Teeth makes a bite attack.

Lair and Lair Actions

(Initiative Count 20, losing ties)

When fighting within its swamp territory, Thousand Teeth can invoke the environment. On initiative count 20 (losing initiative ties), Thousand Teeth takes a lair action to cause one of the following effects. It can't use the same effect two rounds in a row.

Thrashing Wake. Thousand Teeth churns the surrounding water into a violent froth. Choose a 20 ft radius area of water that Thousand Teeth is in or adjacent to. That area becomes heavily obscured until initiative count 20 on the next round. A creature inside the area is effectively blinded with regard to anything beyond the obscured area, and creatures outside the area cannot see into it.

Collapsing Bank. The unstable swamp bank gives way. Choose a 10-ft square of muddy earthen ground within 60 ft of Thousand Teeth. Each creature standing in that area must succeed on a DC 15 Dexterity saving throw or fall prone. The affected area becomes difficult terrain until initiative count 20 on the next round. If the selected area borders water, a creature that fails the saving throw is also moved 5 feet toward the nearest water space.

Undertow Pull. The sudden displacement of water drags creatures off balance. Choose one creature that is standing in water or within 5 ft of the water's edge within 60 ft of Thousand Teeth. The creature must succeed on a DC 15 Strength saving throw or be pulled up to 15 feet in a straight line toward deeper water or toward Thousand Teeth (Thousand Teeth's choice). This movement does not provoke opportunity attacks.
