Expert Senses
Blood Wardens are typically blind due to the rot and gore grown around their eyes. They feed on blood and organs so they've developed a very keen nose to smell these from many miles away. When a Blood Warden smells either of these it will quickly make it's way to the target and attack them no matter what.
Piercing Ram
When using Piercing Ram, the Blood Warden will bow down it's head and ram into it's target using it's long, singular horn. Piercing Ram has a travelling distance of 15ft and can pierce through multiple targets as long as they're standing alongside each other in a straight line. The first target hit by this attack will take 2d12+6 piercing damage. Anyone else hit after this will take 1d12+6 piercing damage.
Raveging Attack
When using Ravenging Attack the Blood Warden will attack it's target three times. It will first attack it twice with it's claws. The target will need to roll a dexterity saving throw for each attack. If >15 they will hit and deal 1d10+7 slashing damage each. The final attack consists out of a horn slam, here the Blood Warden will slam it's sharp horn down on the target, attempting to hack into their head. The target will need to roll a strength saving throw, if <17 the horn will collide with the head and deal 2d12+5 bludgeoning damage.
Blood Cleave
After taking a reasonable amount of damage the Blood Warden will start a blood manipulation ritual with it's own and the party's blood. When all the blood is in the air the Blood Warden will clasp it's hands together and attack every party member with a cleave of blood. The party will all need to roll a dexterity save, if <16 they will be hit with 1d12+8 slashing damage and 1d10+5 blood damage.
Mantle of the Unseen
When using Mantle of the Unseen the Blood Warden will open it's mantle, revealing multiple fleshy eyes grown that are grown into it. This will reveal any invisible players. Everyone standing in it's 20ft range will need to roll a wisdom saving throw, if <15 the eyes will pierce their soul and deal 2d10+8 psionic damage.
Slash
When using slash the Blood Warden lunges at it's target and cuts straight into it's flesh with it's claw. The target will need to roll a dexterity saving throw, if <17 they will take 1d12+5 slashing damage.
Mark of the Warden
When using Mark of the Warden, the affected ally will take twice as much damage. When this ally dies all of it's blood will flow towards the Blood Warden, healing it for the amount of hitpoints the ally had when it was marked.
Dissolve
When using Dissolve, the Blood Warden will wrap it's mantle around itself and either go invisible or teleport to a different area. The teleporting range is 20ft.
Blood Manipulation
When using any blood related spell or attack, the Blood Warden will counter the attack and redirect it onto the attacker.

Description
Origin
The Blood Warden is a tall creature that roams the Bloodlands, always hunting for new prey and blood. They have one long, straight horn on their heads and have a huge pair of sharp claws. The Blood Warden also always wears a Mantle of the Unseen, a mantle made out of black feathers with eyes on the inside. A Blood Warden is usually blind due to all of the rot and gore that ate it's eyes out so it can use these eyes to see. Blood Wardens are also known as the children of the Blood Star, often gaining a small following of worshippers that sometimes even build a church for it. The Blood Wardens came alongside the Bloodlands and were one of it's first apex predator, but as the years went on they've been overshadowed by countless of other horrors. It's motivation is it's lust for blood and hunger, a hunger impossible to satisfy.
