Traits

Fly-by. If the Eyate's first attack is its stinger, it does not provoke attacks of opportunity. 

Area restriction. An Eyate cannot end its turn more than 30 feet away from Flayer the Sane. If Flayer the Sane moves and an Eyate would be more than 30 feet away, the Eyate is forced to move to remain within 30 feet of Flayer the Sane. This does not provoke attacks of opportunity. 

Actions

Multiattack. The Eyate can make a Stinger and Eye Ray attack if possible. 

Stinger. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 2d4 poison damage. Target must make a DC 12 Constitution saving throw. On a failure, target has the poisoned condition. 

Eye Ray. Ranged Attack Roll: +7, reach 5 ft. or range 120 ft.  or take 10 (1d12) force damage.

 

Bonus Actions

Aggressive. The Eyate can move up to its speed toward a hostile creature that it can see.

Description

Eyate's are a unique creation from the Lich known as Flayer the Sane. They always hover nearby and are used as cannon fodder to prolong his life. They are not autonomous and are relatively easy to create, as long as Flayer has a soul on hand.
