Omniscient Dreamer: The Aspect of Azathoth is aware of everything that has been, is or will be across all realities. Nothing can be hidden from it.
Reality Unbound: The Aspect isn't affect by time, space, or the laws of physics. It can't be targeted by effects that require line of sight and it can't be contained.
Unfathomable Form: The Aspect has no true form. It appears as a swirling mass of writhing chaos and incomprehensible geometry. Any description fails.
Dreams All Things: Each round at initiative count 20, The Aspect of Azathoth chooses or randomly determines a reality-warping effect (DM choice)See Legendary Actions
Obliteration Gaze: The Aspect opens a myriad of eyes. Each creature in a 120 foot cone must succeed a DC24 WIS saving throw or be erased from reality. On a success, the creature takes 12D10 psychic damage (66 average) and is banished to a random plane until the end of their next turn.
Dream Collapse: The Aspect collapses pieces of reality. Each creature in a 60-foot radius must make a DC24 CON saving throw, taking 16D10 force damage (88 average) and being knocked prone on a failure, or half as much damage on a success.
Thought Unmaking: The Aspects targets one creature ir can perceive within 120 feet. The target must succeed on a DC24 INT saving throw or have its mind unraveled. The creature is stunned, can't take actions, and forgets everything it knows until the end of its next turn
Yawning Silence (Recharge 5-6): The Aspect unleashes the silence between realities. Each creature within 120 feet must succeed on a DC24 WIS saving throw or be deafened, take 18D10 psychic damage (99 average), and be incapacitated until the end of their next turn.

The Aspect of Azathoth can take 3 Legendary Actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The Aspect regains spent Legendary Actions at the start of its turn.
Dream Pulse: The Aspect emits a pulse of dream-entropy. Each creature within 60 feet must succeed on a DC 24 WIS saving throw or take 8D10 psychic damage (44 average).
Shifting Unrealities: The Aspect warps reality. One creature it can see within 120 feet must succeed on a DC24 DEX saving throw or be teleported to a random location the DM determines.
Void Step: The Aspect moves up to 120 feet without provoking opportunity attacks and can move through other creatures and objects.
Call To Nothingness (Costs 2 Actions): The Aspect summons a rift to the ultimate void in a point it can see within 120 feet. Each creature within 30 of that point must succeed on a DC24 CON saving throw or take 14D10 force damage (77 average) and be pulled 20 feet toward the rift

Description
Azathoth is the nuclear chaos at the center of all existence, the mindless source of infinite universes, He lies beyond time, dreaming worlds into being and unmaking them with a whim. Even The Great Old Ones are but figments within his dream.
Because this is a cosmic boss with a lair, use:
Treasure Hoard — CR 17+
- Coins: 12d6 × 1,000 gp
- Platinum: 8d6 × 1,000 pp
- Gems: 1d8 × 5,000 gp gems
- Art Objects: 1d10 × 2,500 gp art objects
- Magic Items: 1d4 + 1 magic items
For the Aspect of Azathoth specifically, I'd strongly recommend modifying the normal hoard so that the treasure feels like something recovered from the Centerless Void, rather than simply finding a giant pile of gold.

I'd use the CR 17+ table for the baseline, then replace some of the mundane treasure with:
- 1 Legendary or artifact-level eldritch item
- 1–2 Very Rare magic items
- Several strange cosmic gems/relics
- A forbidden tome containing reality-warping knowledge
- Fragments of impossible objects that have no equivalent in normal reality
The most important reward should probably be a unique artifact connected directly to Azathoth, rather than the gold.
Lair and Lair Actions
Lair: The Centerless Void6 floating fragments of The battle takes place in the Endless Dream, a void of swirling stars, impossible angles, and 6 floating fragments of broken realities
- Gravity shifts and bends unpredictably
- Time flows unevenly, rounds may speed up or slow down (DM discretion)
- Fragments of realities drift by, granting cover or unleashing hazards
- At initiative count 20 (losing ties), the Aspect's dream twists the battlefield
Lair Actions
Reality Fragments: Floating remnants provide or unleash random effects when touched
Stars Unmaked: Stars wick in and out of existence, dealing 6D10 force damage (33 average) to creatures in the area.
Mind-Breaking Whispers: Creatures must succeed on a DC20 WIS saving throw or suffer disadvantage on attacks until the end of their turn.
Rifts To Nowhere: Rifts open randomly. A creature ending its turn within 10 feet of a rift must succeed on a DC20 DEX saving throw or be pulled in
