Horrid Scream. The Bone Juggernaut screams in agony with every step, unsettling nearby foes. Any creature that is within 60 feet of the colossus that can hear it must succeed on a DC 12 Wisdom saving throw or become frightened for 1 minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. If a creature's saving throw is successful or the effect ends for it, the creature is immune to the Bone Juggernaut's Horrid Scream for the next 24 hours..
Multiattack. The Bone Juggernaut makes two bone spur attacks.
Trample: If the Bone Juggernaut moves at least 10 ft and hits with a Bone Spur attack, the target must make a DC 14 Strength save or be knocked prone and suffer 2d10 piercing damage. If the target fails the save, the Bone Juggernaut may move through the targets space and the target may not perform an opportunity attack against the Bone Juggernaut this turn.
Bone Spur. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 10 (2d6 +3) piercing damage.
Description
Assembled from bones and scrap, the Bone Juggernaut is a vile creation of necromancers. It walks on four to six long, inscet-like spurs made of metal or bone that are razor sharp which it uses to trample and puncture its prey. The Bone Juggernaut screams in constant agony as its misshapen form scuttles about, driving fear into the hearts of its foes.

Posted Mar 27, 2021I used this in my campaign... and it freaked out my players. I had it have four spider like legs, a long neck made only of ribs, and the skull of a minotaur. to this day I am still proud of that design simply because I freaked out someone who likes horror
