Alternate Form: The Blood Snail's body is like elastic, and can morph into different shapes. When the Blood Snail leaves it's shell, it can transform into a flying snake-like beast. Its AC becomes 14, dexterity becomes 18 (+4), strength becomes 14 (+2). Movement speed becomes: Walk 30ft, fly 60ft, swim 30ft.
Snail Form:
Tentacles: Melee Weapon Attack: +5 to hit, reach 15 ft., one target. Hit: 10 (2d6 + 3) bludgeoning damage. If the target is a creature, it is grappled (escape DC 16). Until this grapple ends, the target is restrained, and the snail can't use its tentacles on another target.
Snake Form:
Bite: Melee Weapon Attack: +6 to hit, reach 10 ft., one target. Hit: 6 (1d4 + 4) piercing damage.
Retreat. When the Blood Snail takes damage, it hides in its shell until it's next turn. It cannot take damage while in it's shell. Any grappled creatures are released. On it's next turn, it exits it's shell to become a flying snake.
