Many Faces. Axihuatl wears a porcelain mask that makes her appear as a Medium humanoid. While Axihuatl is wearing a mask, she has an Armor Class of 17, and a walking speed of 30 ft. Axihuatl has three masks, each granting her one Legendary Resistance:
- Porcelain Mask. This mask grants resistance to all damage apart from force damage, and breaks when Axihuatl has received 30 force damage while wearing it.
- Vi'Poolgol. This mask grants resistance to all damage apart from lightning damage, and breaks when Axihuatl has received 30 lightning damage while wearing it.
- Tōru Kodama. This mask grants resistance to all damage apart from necrotic damage, and breaks when Axihuatl has received 30 necrotic damage while wearing it.
Once all three masks break, she becomes a Gargantuan fiend, with an Armor Class of 15, a walking speed of 20 ft., and a fly speed of 60 ft. Aside from her size, walking speed, and Armor Class, her statistics are the same in each form. Any equipment she is wearing or carrying isn't transformed.
Regeneration. Axihuatl regains 20 hit points at the start of her turn. If Axihuatl takes acid or fire damage, this trait doesn't function at the start of Axihuatl's next turn. Axihuatl dies only if she starts her turn with 0 hit points and doesn't regenerate.
Innate Spellcasting. Axihuatl’s spellcasting ability is Charisma (spell save DC 18, +10 to hit with spell attacks). She can innately cast the following spells, requiring no components:
At will: toll the dead
3/day each: life transference, spiritual weapon, vampiric touch, wall of water
1/day each: destructive wave
Multiattack. While in humanoid form, Axihuatl makes three attacks with Stinger (rapier). Once in fiend form, Axihuatl makes three attacks: one with her tail stinger and two with her pincers.
Stinger (humanoid form only). Melee Weapon Attack: +9 to hit, reach 5 ft., one target. Hit: 8 (1d8 + 5) piercing damage. When Axihuatl attacks a creature with this magic weapon and rolls a 20 on the attack roll, that target takes an extra 10 necrotic damage if it isn't a construct or an undead. Axihuatl also gains 10 temporary hit points.
Tail Stinger (fiend form only). Melee Weapon Attack: +9 to hit, reach 5 ft., one creature. Hit: 19 (4d6 + 5) piercing damage plus 24 (7d6) necrotic damage, and the target’s hit point maximum is reduced by an amount equal to the necrotic damage taken. If this effect reduces a creature’s hit point maximum to 0, the creature dies. This reduction to a creature’s hit point maximum lasts until the creature finishes a long rest or until it is affected by a spell like greater restoration.
Pincers (fiend form only). Melee Weapon Attack: +9 to hit, reach 5 ft., one target. Hit: 10 (1d10 + 5) piercing damage.
Counter-Flank. While in humanoid form, when Axihuatl is hit by an attack that has advantage due to flanking, she can make one attack against the attacker with Stinger.
Axihuatl can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. Axihuatl regains spent legendary actions at the start of her turn.
Cast a Cantrip (Costs 1 Action). Axihuatl casts one cantrip from her Innate Spellcasting trait.
Stinger (humanoid form only) (Costs 1 Action). Axihuatl makes an attack with Stinger.
Cast a Spell (Costs 3 Actions). Axihuatl casts one spell from her Innate Spellcasting trait.
Drone (fiend form only) (Costs 3 Actions). Axihuatl produces a horrid droning sound to which demons are immune. Any other creature that starts its turn within 30 feet of Axihuatl must succeed on a DC 12 Constitution saving throw or fall unconscious for 10 minutes. A creature that can’t hear the drone automatically succeeds on the save. The effect on the creature ends if it takes damage or if another creature takes an action to splash it with holy water. If a creature’s saving throw is successful or the effect ends for it, it is immune to the drone for the next 24 hours.
