Slippery Beasts. The agnathe has advantage on checks to escape a grapple and can't be slowed down by difficult terrain. While moving, agnathes can slither through holes of size small or larger.
Dark Swimmers. The agnathe has advantage on Dexterity (stealth) checks while underwater or in darkness.
Multiattack. The agnathe makes five attacks: one with its bite and four with its tentacles.
Bite. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 12 (1d8 + 2) piercing damage and the agnathe's mouth attaches to the target. While attached, the agnathe doesn't use its bite attack. Instead, at the start of each of the agnathe's turns, the target loses 2 (1d4) Constitution points due to blood loss. The target loses a number of hit points (as well as an equal number of maximum hit points) equals to its number of Hit Dices time the Constitution modifier it lost.
Tentacle. Melee Weapon Attack: +6 to hit, reach 10 ft., one target. Hit: 5 (1d6 + 2) bludgeoning damage plus 4 (1d8) poison damage.
Description
Agnathe are enormous, monstrous leeches with a never-ending thirst for blood. They have a large round body which extend into an elongated neck with a round sucker mouth. Four dark tentacles can extend out of the creature's sides and attack any surrounding hostile creature. While not inherently evil, their vicious pack tactics and the atrocious wounds they leave on their mangled victims give them an unsavory reputation.
Silent slithers. Despite their size, agnathes are remarkably stealthy while on the hunt, often focusing on the biggest, juiciest target within a group. They prefer foggy swamps and dark underground lakes, where they can hide and hunt in shallow muddy waters.
