You and up to five willing creatures within 5 feet of you instantly teleport to a previously designated sanctuary. You and any creatures that teleport with you appear in the nearest unoccupied space to the spot you designated when you prepared your sanctuary (see below). If you cast this spell without first preparing a sanctuary, the spell has no effect.
You must designate a sanctuary by casting this spell within a location, such as a temple, dedicated to or strongly linked to your deity. If you attempt to cast the spell in this manner in an area that isn't dedicated to your deity, the spell has no effect.
You can agree any time before hand whether a preson is willing. Thus, if they go unconscious it has already been established they are willing.
"...within 5 feet of you..."
So, I would;
1) establish within the party well beforehand whether characters are willing so if they go unconscious there is no doubt
2) establish beforehand a rallying cry such as "recall....rally around me" so the party gets within 5 feet and you can cast the spell.

I'd rule that an unconscious party member is willing if they WOULD have been willing if conscious. That makes sense RAW become the spell doesn't specify that the person must be conscious, and 5e states all such exclusions explicitly rather than allowing them to be implied or inferred. Also, party members would probably have discussed the use of the spell previously.

You have to cast this spell to designate the "sanctuary" and it has to be in a location strongly linked to your deity as specified in the spell text.
Doesn't say it can only be one place. But it has to be linked to your deity in some way.
