Base Class: Barbarian
We get it, you work out a lot.
Adrenaline
At 3rd level you gain Adrenaline.
- Adrenaline points are earned one per round in which a character makes at least one attack.
- Whenever adrenaline points reach a certain threshold, the character’s adrenaline bonus goes up. (1-4 points is +0, 5-9 is +1, 10-14 is +2, etc.)
- The adrenaline bonus applies to several types of roll. (include hit chance, saving throws, and ability checks)
- Whenever you take a reckless attack action while raging, you gain an additional Adrenaline point.
- The character loses one adrenaline point per minute outside of combat
Too Many Gainz For My Brain
At 3rd level, you gain Too Many Gainz For My Brain
- During a short rest, you may spend 3 Hit Dice to remove one level of exhaustion. Hit Dice spent in this way are not rolled and cannot be used to regain Hit Points.
- Make a DC 10 CON save to recover 1 exhaustion level.
- Finishing a Long Rest automatically reduces a character’s exhaustion level by 1, provided that the character has also ingested some food and drink.
When at 2+ exhaustion
You can suffer a level of exhaustion due to a mental burden (DM or players choice). Charisma [public embarrassment, etc.], intelligence [thinking strain, etc.] or wisdom [being disguised for long periods of time, etc.]. The character must succeed on a DC 10 (mental) saving throw or suffer a level of exhaustion.
When at 3+ exhaustion
When taking a long rest, roll percentile dice. This percentage roll is for nightmares (20% base +10 for every time you were knocked unconscious). If a player has a nightmare that night, then they do not get a long rest and they receive one point of exhaustion.
You can reduce the nightmare percentage by 2% for every long rest you have, down to the base 20%
Risky Business
At 3rd level, you gain Risky Business
- Add a level of exhaustion when hit by a critical hit (Melee, ranged, spell).
- Add a level of exhaustion when taking more than 1/2 Hit points in a single round.
- Add a level of exhaustion when reduced to 0 hp.
- Every time you fail a death save, you get a level of exhaustion (you don't die at 3 death saves, you die at 6 exhaustion, 8 exhaustion at lvl 6)
- When a character who was knocked unconscious regains Hit Points and returns to consciousness, that character must roll a D20.
- If the character rolls a 13–20, they do not suffer anything.
- If the character rolls a 6–12, they suffer another level of exhaustion (so, you can get 2 levels total)
- If the character rolls a 1-5, they will instead roll on the lingering injury D100 table. (roll twice and choose which injury makes the most sense)
- The number on each effect increases by 2 for every subsequent time that the character returns from unconsciousness before the end of the encounter.

Lingering Injury:
https://worldbuilderblog.files.wordpress.com/2015/07/lingering-injuries.pdf
Well, I F**cked up
A character may sustain a lingering injury after rolling a natural 1 on a saving throw against a spell, magical, or trap effect that deals damage to the character.
Adrenaline Rush
At 6th level, you gain Adrenaline Rush
Using a bonus action, you can choose to start an Adrenaline Rush. The rush remains active until combat ends or 1-minute passes outside of combat. During the Rush, movement speed is doubled with advantage on strength checks/saving throws and +1D4 damage on a successful hit (at lvl 10 it becomes +1D6, at lvl 14 it becomes +1D8).
After the rush, you suffer 2 levels of exhaustion.
Once you use this feature, you must finish a short or Long rest before you can use it again.
I Still Got More In Me
At 6th level, you gain I Still Got More In Me
Improve exhaustion:
Exhaustion Effects
(1) Disadvantage on mental Ability Checks
(2) Speed reduced by 10
(3) Disadvantage on Attack rolls and Physical Ability Checks
(4) Disadvantage on Saving Throws
(5) Hit point maximum halved + 1D6 + barbarian level
(6) Speed reduced by 30
(7) speed reduced to 0
(8) Death
Half Time, Down But Not Out
- At 10th level, your soul is marked for endless battle. When you take damage that reduced you to 0 HP or suffer and effect that would cause you to die while raging, you may instead use a Rage to return to life with 1 HP and gain temporary HP equal to 1/2 your maximum HP value that lasts for 1 minute. You will also be exhaustion level (5). Once you do this, you may not use this again until you have completed a long rest. [Exhaustion 5 = hit point maximum halved + 1D6 + Barbarian level]
- Increase death saves to 4.
Solution To Enhance Resistance Or Increase Direct Strength (S.T.E.R.O.I.D.S.)
At 14th level, you gain Solution To Enhance Resistance Or Increase Direct Strength (S.T.E.R.O.I.D.S.)
This small vial of greenish-blue liquid has a long, thin metal piece on one end, and a plunger on the other end. It contains one use of S.T.E.R.O.I.D.S. Injecting the concoction into a creature requires an action.
The vial may be refilled, but requires 100gp worth of materials and an Alchemy check of DC 15 (or 20).
Upon the first use, you may choose to gain either resistance to one damage type (acid, cold, fire, lightning) for one minute, or to increase your Strength score by 2 for one minute (Your maximum for Strength is now 22). Using S.T.E.R.O.I.D.S. more than once per long rest is dangerous. For each additional injection, the creature must roll a Constitution saving throw. The DC for this throw is 10 + the number of times that creature has been injected. On a failed save, the creature gains one level of exhaustion and must roll a Wisdom Saving throw:
On a 1-5, you go into a blind rage, attacking the closest being near you, continuing until a minute has passed; then take 2d10 necrotic damage, take another level of exhaustion and pass out (passing out does not count as a long rest in this case).
On a 6-10, you pass out and take an additional level of exhaustion and 2d8 necrotic damage (passing out does not count as a long rest in this case).
On an 11-15, you take an additional level of exhaustion and 2d6 necrotic damage.
On a 16-20, you take 2d4 necrotic damage.
On a 21+, nothing happens.
