I had a player create 3 characters in my campaign. I'm not sure how that happened, but it did.
So in my campaign's list of characters, under "Active Characters", I saw 3 characters from the one player.
However, when I view one of the character's Character Sheet, and click the campaign button next to the "Forge" (go to builder), only one of that player's characters appears in the list. I think this one was the first character the player added to the campaign.
I then removed two of that player's characters from the campaign (including the one that was displayed from the sheet).
Now I only see one character from the player. But returning to any character sheet, and clicking the campaign button again, the character that was removed from the campaign is still listed, and the one kept in the campaign is not.
Anyone who has access to the campaign can “create unassigned character” whenever they want to. So any player could create multiple PCs.
The behavior you are describing where only some characters are visible when you campaign name occurs when characters are set to “private.” Public Characters will appear in that list, private ones will not.
Your player most likely was experimenting with various builds, and set one as Public for use and kept the others as private assuming that nobody would be able to see them. They likely didn’t realize that everyone the campaign can see them listed, just not view the actual sheets, or that you as DM could still see everything.
