So action surge states that for that turn you you get an "extra action and a possible bonus action". Im confused as to what that means. Does it mean DMs choice, players choice, or is it situational.
It is player choice. Action surge allow you to use 2 actions (regulat action plus 1)and 1 bonus actions on the same turn. Action surge does not allow you 2 bonus actions, just the usual one.
I must admit the answer still does not help. What does the "possible bonus action" mean? I can attack, and action surge a second attack plus...what exactly?... as a "possible bonus action?"
Possible bonus action means that it is not always possible for a character to take a bonus action. If you can, with a spell or other feature, then you can take it.
Not every character can take a bonus action every round - it depends on the classes and feats the character has, and the current situation.
It's a semantic argument, but I would say everyone "has" a Bonus Action, but not everyone can use a Bonus Action every turn.
Not everyone can use all of their actions every turn.
Example: Everyone has 1 ReAction, but until something triggers the ability to use it you can't, but you still have 1 ReAction and therefore once you use the ReAction it's used up.
On your turn, you can take one additional action on top of your regular action and a possible bonus action
What that means is that you normally have a regular action and a possible bonus action (depending on whether you get one or not) and then you can take one additional action (as Filcat said, you still only get one bonus action).

On your turn, you can take one additional action on top of your regular action and a possible bonus action." I almost feel I need a clarification of what a regular action and an additional action is. It seems to me that if I used Action Surge at 5th level, using two weapons, you could attack the same creature 6 times...is this correct?

Five times:
1) regular action: Extra attack ( 2 attacks).
2) Action surge (Additional action): Extra attack (2 attacks)
3) Bonus action, if you are doing two-weapon fighting (1 attack).

Thanks. It is really fun to see people reply to my noob questions from nearly 3 years ago...I still have lots of new questions but I got the answers I need to this one a few years back. Lol. Incidentally, I had been using spiritual weapon wrong up until a few months ago because my DM, misunderstood the rules.
OK, so I'm confused. Everyone seems to agree the Barbarian/Fighter combo gives you five attacks.
First, two-weapon fighting: you use your bonus attack to make a second attack. It's not "two attacks per action". It's one attack as an action, and one attack as a bonus.
Add dual-wield style says the weapons don't have to be light. It says nothing about an extra attack.
Action surge says you get another action, once per long rest. So what my barbarian would get would be:
* One main attack with a battleaxe
* One bonus attack with a battleaxe
*One surged attack with a battleaxe.
So with both of those, I could three attacks... and that's only because I surged. Most of the time, I'd be making two attacks, like anyone with two-weapon fighting. What am I missing???
At fifth level Barbarian (fighter 2) , your attack action gives you two attacks. Then you action surge and take the attack action again, so you get two more attacks. Then bonus action for your off hand weapon. Five attacks total.
And by the way, extra attacks do not stack. So if you go to barb 5/fighter 5, you don’t get a third attack. Only fighter 11 gives you three attack per action.
And just FYI the current wording of Action Surge is much less confusing:
"Starting at 2nd level, you can push yourself beyond your normal limits for a moment. On your turn, you can take one additional action. Once you use this feature, you must finish a short or long rest before you can use it again. Starting at 17th level, you can use it twice before a rest, but only once on the same turn."
I do wish they'd given Action a more specific name, like Standard Action or something, to more clearly differentiate it from Bonus Action.

Different classes, abilities, feats, and whatnot allow Bonus Actions.

Also, if you are an Eldritch Knight, you can actually cast two levelled spells in a round using Action Surge.
