If you cast a spell from the 1st level spell list at 3rd level, you aren't casting a 1st level spell. It's a 3rd level spell from the 1st level spell list. This is true for all class features.
Very True. In general a spell is considered the level it is cast at, not the level it is listed. The exceptions to this rule (as there always are), have to explicitly state otherwise. For example, from Globe of Invulnerability.

Same realm as heal but using life transference. If you twin this spell to heal a second person does give the save value of heal to both targets or split the one value of heal between both? Also does the cater l cater take the necrotic damage twice since it's being cast on two targets?
I have question regarding twinned spell. If I went twin cast a spell, could I also put distant on it as well?
Ex. If I used twinned spell on revivify, could I also use distant so that the revivify spell had a range of 30ft?
Short answer, no; you can only normally use one Metamagic per spell, the only exceptions are Empowered Spell and Seeking Spell, which can be used in addition to another Metamagic.

Hmm, I'm assuming your intention is to also twin silvery barbs if necessary (so you could target both)?
I think strictly speaking each saving throw would be its own, separate trigger, regardless of timing, so you would only get to make a single reaction to one of these. Also the trigger for silvery barbs is specifically "when a creature you can see within 60 feet of yourself succeeds on an attack roll, an ability check, or a saving throw", and the spell itself affects "the triggering creature", in both cases there is only one creature at a time, so even if you could twin it it would only have one possible creature to affect.
However this is still a very powerful combo (arguably too powerful for a 1st-level spell, which is why my group uses a more balanced alternative) because you only need to cast silvery barbs when/if one of the targets succeeds on the saving throw. And you can still combine it with an ally or yourself setting up bane, or a College of Eloquence Bard's Unsettling Words etc. to further interfere with the target(s)' saving throws.
