I was wondering if I had paired it with something such as a familiar or clairvoyance to further the range of teleportation?

It is "a known place", not "a place within view". Teleportation spell can teleport up to eight people within range. If you can see that place from your familiar's eye or with help of any divination, it works fine.
If you can see the location you wish to teleport too, then surely it would then count as a place you know (distance depending)? As example, if you could see from a tower to the road leading away, a Teleport spell would work as a known location as you can see and know the details of such a location well enough for the spell.
So, Both would be the answer I would give if you can see a location clearly.

There's a table for that. You can check the description from Spells section. As in your example, there will be some consequences.

Sure you can, if there is no any story-based condition, like a place made of illusion.
