When creating a custom item, you are able to add advantage to initiative directly, but not disadvantage to initiative. My reasoning is that I use beyond20 and use the auto roll advantage function, which messes up initiative rolls. However, it would be a fun cursed item option as well. I assume this won't get tons of interest from the community, but my hope is it would be an easy change given the existing options.

As far as I am aware, there is nothing official from WotC that would permanently add Disadvantage to initiative, and temporary effects are not currently supported on DDB.
FYI: Pleading Beyond20 interactivity will get you less than nowhere around here. Their opinion regarding such issues is “serves you right, that’s why we tell people not to use Beyond20.”
Beyond20 is a 3rd-party browser extension. It has no official ties to DDB at all, and it causes no end of headaches for the Devs here. Which is part of why they “strongly suggest” to not use it. The other reason they so "sternly advise” against using Beyond20 is because exporting from DDB to anywhere else by any means, including through Beyond20, is actually a violation of the terms of service we all agreed to when we made our accounts.
Technically speaking, everyone who uses Beyond20 could have their account closed, and all licensed content revoked without refund. Unfortunately for DDB, so many people who pay to use DDB also use Beyond20, that they would basically be committing corporate suicide if they did that, so it’s one of those “illegal but unenforceable” things. (Much like most foreplay is in PA thanks to some very antiquated and comprehensive sodomy laws. 🙄)
