For anyone still experiencing this bug, I found a workaround that avoids the 20 minute wait similar to what LiamSiochsturm mentioned. I tested this on two character sheets in dndbeyond after my initial accidental fix.
Note: New PCs set immediately to Custom Background will not receive this bug.
Existing PCs that had a previous background that then changes to a Custom Background will encounter this bug. It will occasionally default to the most recently selected Background or remain blank in the Description setting along with any associated skills.
Workaround: set new Custom Background and then go make any change (remove or edit) to a Proficiency in the Class Features the view sheet. I believe this fix works due to how the Proficiencies are tied between the two data items (Background and Class) in order to inform players when they are picking the same option in either or section. Since the Custom Background is not a standard option, the system may not have an algorithm to be able to easily register the new user defined settings. By removing or making a change, it affects/updates the cache as an earlier individual mentioned that then in turn resets the relationship between the two data items to be displayed correctly.
Hope this helps anyone still struggling with it like I did till now.

Hey, most of a year later than the last post, but I still see this problem. Had it happen before. It seems to randomly resolve after some time, but there should really either be an explanation or a fix for this given that it is a bug first reported in 2020.
Just adding another report of this issue, both myself and a player have experienced it. I've also confirmed that it's affecting a brand new character that never had a previous background. The workaround posted previously is not working either, even changing the class proficiency to one selected by the custom background is ineffective at resolving the problem. This is disappointing that there are a number of reports of this bug over several years with no acknowledgement.
Experiencing this today (12/14/23). None of the suggested fixes (except waiting, which I haven't tried) are working. About all I can do is manually add the proficiencies in the char sheet.
Right. So expecting it to get fixed is pointless. It’s an issue caused by the siloed servers, takes a 15-20 minute cache cycle. It is what it is.
That seems like an odd conclusion when most other updates to the character sheet do not take 15-20 minutes.
Certain bugs come up over and over again in this forum and have been ignored for years. This one, the one with spell feats like Fey Touched and Shadow Touched not letting you cast using spell slots, the inability to replace subclass spells for Aberrant Soul or Clockwork Soul sorcerer...
I'd hope with so many paid subscribers they might be able to afford to hire a person or two to fix some of these ancient bugs. "It is what it is" is one thing when there's no money changing hands, but with paid customers leaving known bugs unfixed for 3+ years is just shoddy business.

Feats are so hard coded in how they function that changing them requires virgin sacrifices. The Sorcerer issue would be relatively easy to fix if DDB was legally allowed to alter the subclasses, but they’re not, so they can’t.
Trust me, I’ve been around here longer than some of these bugs. I know what’s what on a lot of these issues. Trust me, I want them fixed more than anyone so I can stop being the bearer of bad news to folks like you only to have to keep reexplaining the same stuff over, and over, and over, and over again.
