There are a few things you could do that I'd see as appropriate answers to this question: Double down on a strength, shore up a flaw, or just grab something fun.
I'd probably grab something that shores up a flaw, of myself or someone close to me. For example, an Amulet of Health (set CON to 19) could be *really* useful long-term, and could be a literal life-saver to one of my best friends.
For doubling down on a strength, I'd probably go with a Manual or Tome related to what I do day-to-day; for me, one of the Tomes would be perfect (though I can't say which one I'd prefer, the Tome of Clear Thought would be the obvious choice for boosting up a strength).
For something that's just fun, an Amulet of the Planes would be fun, but *extremely* dangerous due to the whole chance-of-failure thing, though hopefully you'd be able to come back to your home plane without too much trouble after multiple uses. A Hat of Disguise or Helm of Telepathy could similarly be fun to use, but a Helm would really stand out in modern-day and while reading other people's thoughts is novel, it isn't particularly *useful* for my day-to-day life.
I'm sure there are others I haven't thought of, and if anything could give me Expertise or Advantage in the Computer Programming or Project Management skills I'd definitely take that over anything else; it'd just be too damned useful.
Or, the absolute best thing, would be something that can create a Simulacrum. That'd be great. Don't think anything that can do that is less than Legendary, though.
If we can bypass restrictions like needing a Dragonmark the Bag of Bounty would be great at keeping costs down.
Cloak of the Manta Ray swim fast and breathe underwater? Suddenly, a whole new world opens up for you to look around.
Helm of Teleportation - with a bad back, unable to drive, and very limited funds for travelling just in my own town let alone anywhere interesting this item would improve my life a great deal.
Rod of Security could be great if you just need a place to chill out. It's basically a free holiday to paradise every 10 days.
Tome of Clear Thought because I value my intelligence and improving that would be fantastic.
These are my top picks. I would be equally happy with any of them.

These are the items that would make the biggest impact on my life on a day to day basis. I'd love to be able to fly or teleport, but that wouldn't affect my daily life very much.
I used to teach acrobatics to children which meant kneeling a lot, shoulder issues, and a back that’s out of place. I’m assuming that it would fix all of those.
Hmmm. Flying carpet, Censer of controlling air elementals, Figurine of Wondrous power... so many to choose from! Periapt of Health to never worry about disease again, even an Immovable Rod would be a lot of fun! I'd probably go with an item that gave me a buddy I could have for life though. For that reason probably one of the Figurines. Maybe the Mastiff, or the Raven.

If you could have one item from the DMG in real life (no legendaries or artifacts) what would it be and why?
For me probably alchemy jug due to unlimited booze, water, and mayo.

I would probably go with Nolzur's Marvelous Pigments and there is an unlimited amount of fun I can have with that. :)
