Immensely power magic user who wishes to collapse the universe and reform it in his master's image
An alternate version of one of your characters, the entire party, or even a key NPC that went evil and wishes to rule, destroy, or just terrorize the worlds. Maybe use Batman Who Laughs for inspiration, or Kang.
A cosmic being from the Far Realms that is attempting to transform the multiverse into chaos and nothingness.
Just a random Joe who got caught up with the Multiversal mob or something and the party was lied to and is hunting this guy for absolutely no reason, just the Mob making others do their dirty work.
Barry Allen, but not. Pretty much a joke on how often and significantly Barry messes up the DC Multiverse, but with DND and not copyrighted characters

If you dont wish to do the whole "multiverse at stake" trope but instead wish to have a realm hopping boss, I have a few Ideas.
A fey creature that can go in and out of worlds on a whim, manipulates his various surroundings in his favor, turn the local wildlife and people against the party.
A lich or Archmage that goes in between worlds to escape the party, ammasing their immense knowledge to hinder the party with both spells and whatever planar tools are at the ready.
A reality warping fellow Like mr mxyzptlk, who hops around and causes chaos among the planes.
A plane hopping dragon
If you wish to do the multiverse as in different variations of the world you made, than I also have a few more Ideas.
A mage amassing multiple different universal versions of himself.
A warrior who goes into different dimensions to escape his opponents.
A gnomish wizard whos magic is damaging the weave, but will not stop traveling, like the spot from spiderman.
Lower/mid level, you could do the Grim Jester (CR11) Perfect chaotic plane hopping evil entity.
Little higher level you could use an Eldritch Lich (CR 15). Seeking out new bodies to inhabit, powers to possess, worlds to spread into and infect.
At even higher levels, the new Arch-Hag might be cool. Leading a coven of witches pursuing some unknown or forbidden power. Fueling their planar travel with the blood of innocent bystanders.
I am running a D&D campaign where the players follow the BBEG around the multiverse, but I don't have any ideas. Do you have any ideas?
1. A master thief with an Amulet of the Planes out to steal the most powerful relics or rarest/uniquest items from every plane while framing different people for the theft to create conflict and chaos and become the Champion of Mask.
2. A mindflayer-like creature that desires to feed on increasingly intelligent creatures to become the most intelligent being of all time and destroy the gods with the power of pure logic and reasoning. They travel the planes in a special ship / capsule which they continuously upgrade with golems / constructs they invent as they become more intelligent by feeding off different lifeforms from each plane they visit. [Alternatively, they might be some kind of shape-shifter and are absorbing the best/most powerful abilities of creatures from every plane]
3. A "collector" from the Far Realms who is travelling the planes collecting specimens of every species/creature to bring back to create a zoo / natural history museum in the Far Realms.
4. A god-like entity who has made a bet with another god-like entity that they can travel to every realm in [choose some amount of time] and must collect a 'souvenir' from each realm to prove it. However, each time the entity crosses planes it rips a hole in the fabric of the worlds and monsters from both sides start wandering through and creating chaos.
5. The servant of a chained god, who must travel to each realm and destroy the magical seal found in that realm keeping its master bound.
