Edit: Here's one that was cleaned up enough to be presentable :) The Ring of the Messenger - created for a 3rd level Party in a low-magic setting, who didn't have a Message spell amongst themselves.

Once made a circlet that very simply allowed double-concentration. Figured since they were nearing the end of the campaign, what harm could it do?
Oh was I ever mistaken! That item alone nearly turned our sorcerer into the next campaign's villain.
