I recently commissioned a sweet illustration for one of my homebrew items (https://www.dndbeyond.com/magic-items/2730086-eversmoking-bong), and I'd like to add it as the item image on the homebrew page, to replace the default image, which is currently some sort of lobster submarine.
Is there a way to upload custom images for the homebrew things you create?

yes you must download to image to the portrait slot doing this will replace the default portrait and add the image into the description

Found it! In the Edit page, it's under the "Additional Information" heading, items Avatar and Large Avatar.
