Assuming no one objects to the plan Damian steps back and hides to the west by the tapestry and motions for Remi to open the door. Stealth: 5 Damian then utters an infernal word and makes a quick gesture with his hand, a soft red light emanating from his ruby ring. (Misty Visions) Then, suddenly, another Damian appears by the door facing the corridor south. "I am here to apprehend any and all cultists on orders from the Flaming Fist, walk out with your hands on your heads or you will all be incinerated momentarily." Damian says clear and loud. Deception: 23 rolled with advantage if helped by illusion, otherwise go with first roll.

The skull flail whistles through the image and the figure pauses in its backswing, growling a confused, "Eh?" Before whipping its head east and west.

Ankita lifts her staff up and breathes a whisper casting Shillelagh on it, giving it a soft blue glow and stalks over to the Necromite and swings it from above hoping for a crushing blow.

Ankita - Your staff bashes the man across his shoulder and he grunts in pain. Surprise doesn't automatically grant advantage, it just gives you a round to act before anyone that you've gained surprise on. Your first roll was still high enough to hit though.
Remi - How do you want to do this?

NoM 2 will rush forward into the small room and attempt to bash Rhogarr with its skull flail. It rebounds off of his breastplate with a CLANG.

Thank you for pre-posting while you had the chance :) Also, when I look at your character sheet it says you have a +3 bonus to damage rolls with your rapier, but I noticed you subtracted 1 from the damage in the roll. Am I missing something on your sheet or was that a mistake? If it was a mistake we will go will 9 for the damage instead of 5.

It was deliberate. Reckless Attack works only with strength attacks for to-hit and damage but D&D Beyond always uses the best available mod for versatile weapons. The +3 applies when using Dex, but with Str, it's -1, but with advantage. In addition to reckless attack giving advantage to the 'to-hit' it also gives advantage to those attacking him, which I should have explicitly called out in my previous post.
Damian - The crazed worshipper of Myrkul explodes from the force of the eldritch blast, showering Remi and Rhogarr with gore and rags. He never knew what hit him.
Rhogarr & Rah'zher - I will consider your attacks Readied to be triggered if an enemy moves within melee range.
All - The room beyond is as dark as the rest of the dungeon, but you can see something on the floor of the room just at the edge of the torchlight.
Samuel - Moving closer you see there is an unlit torch lying on the floor next to the shape. With a snap of your fingers it catches fire and illuminates the room, revealing another cultist laying on the floor. They aren't moving and you aren't sure if they're alive or not.

When Samuel lights the torch in the proceeding room, Ankita sees the cultist laying on the floor motionless. She grips her staff tightly and readies it to attack if the cultist attacks her, but she creeps into the room, walks along the East wall until she is straight across from them. (I think I have the motion for that, or however far she can get to that spot along the East wall)
Then she says in a soft voice to not alert those down the stairs next to the cultist, "You on the floor, state who you are and your business but do not move or you will be killed like your allies."

Ankita - As soon as you enter the room the woman on the floor screeches and stands! She gibbers madly and extends her filthy hand toward you as she retreats into the western passage, "Your bones for the lord of bones!" Shadows gather around her hand and suddenly lash out at you in the shape of a large skeletal claw. The claw tears a number of brutal gouges in your front and she cackles as your blood spatters the floor.
Attack: 24 Damage: 7
You get an attack of opportunity as she retreats down the stairs.

With his darkvision, can Samuel identify what the object is? Are there any torches on the wall inside the next room which are unlit?

If there is a visible threat then Samuel will move in to attack with his rapier.
