Sorry I missed 'upcast' (I am usually just glancing at my phone when I have a minite or two. Its not an excuse, but I do know what my issue is. Well, rolling dice boosts engagement, double for playong facing rolls, so thsts my preference, but i guess this will jave to be a DM roll.)
The Invisible Inquisitor casts counterspell on Trisk's counterspell as a reaction (on the inquisitors' turn, before Trisk casts slow on his turn) at 3rd level, so she will need to beat a DC of 16, to allow the cone of cold spell to go off

Ethan still has his reaction so he tosses a harmless eldritch baseball into the inquisitor's face.
(OOC: The baseball is actually 5th level Counterspell, like you know when you hit someone on the head to interrupt their focus; Since it's 5th level, if the spell is 4th level or lower, it instantly fails)
At Higher Levels. When Ethan cast this spell using a spell slot of 4th level or higher, the interrupted spell has no effect if its level is less than or equal to the level of the spell slot he used.
The target is invisible. Conditions for casting counterspell (under casting time) require seeing the creature within 60ft casting the spell.

Not with greater invisibility, I thought. Checking, I see it does need concentration but is not interrupted by casting or attacking, as invisibility is.
The Invisible Inquisitor runs straight for Sam and Trisk, casting a spell as she arrives at the spot between the two. She vanishes from Tetrevess' view as she completes her spell.
The rat becomes invisible and walks directly toward Tetevess, stopping right in front of him, just behind Triskellion.
Tbe scorpion dropped by the incapacitated inquisitor shakes as it stares at Ethan, brandishing its venom tipped stinging tail (make a DC10 Wis save or become frightened for 1 minute). It then walks toward the party, weaving left around Trisk, coming to rest between Sam, Tetrevess and Triskellion.
The other inquisitor shifts slightly to the left (no AoO) casts fireball, targeting Samogri, Teisk and Ethan, but because of the effects of the slow spell, it takes unusually long to cast.
This post has potentially manipulated dice roll results.

He quickly starts wailing on her, attacking twice and then spends a ki point to attack twice more with Flurry of Blows, focusing all attacks at her head. He will spend ki points to use Stunning Strike to stun his target.
<ooc: he can spend one ki point per hit, so she needs to roll DC 18 CON saves up to 3 times>

A ghostly, skeletal hand appears inches in front of inquisitor 4 and if hit the ghostly, skeletal hand will cling to the front of the inquisitor's clothes, and it can't regain hp until the start of my next turn.

As Triskellion casts a spell upon the battered inquisitor, he attempts to summon an ephemeral arcane shield. Unfortunately for him, still feeling the effects of Trisk's slow spell, he is unable to react in time. The grim hand seizes the inquisitor and a pall sets over his flesh as he crumbles to the floor.
I thought that one was the most narratively satisfying so I went with it, even though it posted last. I don't regard the initiative as a set order; it's just before or after NPCs.
So, with that in mind, the attacks from everyone else are targeting a dead guy. If you want to just change the target, that's fine. Or just rewrite it.
You can hear footsteps and shouting from uostairs as armored Troopers mobilize a response, evidently alerted to tbe commotion in the cellar.
