Werewolves, undead, anyone carrying a bunch of bones, anyone possibly approaching the main entrance to the orphanage with no good obviously on their minds.
Lucento takes note of the dirt covering the floor and the shovel at the base of the bed, but is careful not to allow his face to betray his suspicions. First things first.
Lucento walks to the bedside and places the back of his hand on the boy’s forehead. ”Headmistress, you say his condition has worsened slowly? When did this begin?"
From this closer distance, Lucento scans the boy’s symptoms to see if he can further discern the nature of the affliction.

It began about a week ago. It's been a steady decline the whole time.
You notice some bruising on his skin, too, but that's it. Your limited medical training is not helping you here; you can't tell what his condition might be.
Lucento raises his right hand to his chin. A week ago would place the onset of the illness to around the time that St. Andral's sacred remains were removed from the nearby temple. Lucento gradually covers his mouth with his hand, and quietly utters a short incantation. [Casting Detect Evil and Good]
"Headmistress, I regret to say that this is no mere illness. His fire is weak, and dimming rapidly. It is plain that you care for this boy. I'm afraid that my ability to rekindle his flame requires that I ask some uncomfortable questions and that you answer me candidly. I apologize for being so forward, but there is a life at stake and little time."
Her eyes narrow when she notices you attempting to conceal something, though she may not know quite what it was. And do you work alone, Lucento of Lethander?
You do not detect evil or good aberrations, fiends, etc., in this room. It's a concentration spell, and lasts a minute.
Got it. Just to clarify, the spell should also indicate whether any consecrated or desecrated objects are within a 30 foot sphere of where Lucento is standing and not otherwise concealed by (1) 1 foot of stone; (2) 1 inch of metal; (3) a thin sheet of lead; or (4) 3 feet of wood or dirt.
"Headmistress, I cannot stress enough the urgency of this situation, but if it eases your concern I am happy to tell you that I am travelling with friends, a gnome, a dwarf, and a halfling, whose aid we may very well need." Lucento places deliberate emphasis at the end of his sentence, hoping that Tulip -- and by extension Barnaby -- will catch his meaning. "But my incantation has confirmed what I most feared -- your ward is at the brink of death, and it is unlikely he is at the mercy of any mere germ. Madam, please, do you understand that we must act quickly, and that I need both your help and your candor if he is to be saved?"
Gone is the upset woman who you first encountered. She is now the stern headmistress of an orphanage, ready to deal with more uninvited guests. If that is your team then go and fetch them. I do not need your Lethander to tell me that Milivoj is not well.
To allow a flame go out is no different from dousing it oneself. She may very well not need Lethander to tell her that Milivoj is ill, but she will know Lethander by his blessing or his fury before I depart from this place or these children. The God of the sun burns brightest when the flames on earth are small.
"My apologies, headmistress. I assure you I meant no offense; I neither expected to need the others nor did I want to presume that so many strangers would be permitted. But very well, my companions should be in the streets nearby, and as you wish I will go down to the entrance and call for them."
Lucento bows to the headmistress, while stealing a glance a Milivoj, the dirt covering the bedroom floor, and the shovel at the base of the bed, before heading downstairs where he stands at the threshold of the orphanage and calls out for Kif, Barnaby, and Fope.
Just before Lucento passes through the front door of the orphanage, he walks in front of double doors to his left - to the right of someone entering the orphanage; on the opposite side of the hallway from the large room where children were playing - and he senses the presence of something evil in the room beyond those closed doors.
Headmistress Belasco stands behind him in the hallway, watching imperiously as he steps into the daylight outside.
The party regroups outside the orphanage, in the street.
Barnaby, this conversation is at the edge of your field of view, unless you and the others move (by entering the orphanage, or whatever).

What are you trying to see?
Just so I know where you are trying to position yourself.
Werewolves, undead, anyone carrying a bunch of bones, anyone possibly approaching the main entrance to the orphanage with no good obviously on their minds.
Ok. You hide yourself in an alley with a good view of the front door.

Both happened "about a week ago," as I read the posts.
Make a sleight of hand check.
Yep, that was my mistake. I went back through the older posts and "edited" Lucento's thought process accordingly.

Yes, that's correct. Nothing triggers it at this time.

Lucento, what's going on?
