The land druid's nature's ward feature says: "When you reach 10th level, you can’t be charmed or frightened by elementals or fey, and you are immune to poison and disease."
My question is: Are you immune to poison damage, the poisoned condition, or both?
I read it as: "if the cause is a poison, you are immune to all its effect."
So, I think it is both, damage and condition.
I'd also say it's both. So if it is an effect delivered by poison or actual poison damage you're immune to it. Personally I rule the resistance to poison of dwarves the same; first they get advantage on the saving throw for half damage and then the damage they received after the saving throw is also halved.
