Looking at the artwork of Vecna that came with the dossier, he appears to have both eyes and both hands, which I thought were supposed to be missing. Is this an oversight by the artist, or am I missing something?
It appears the hand in the art could be artificial, but the eyes look the same.
Probably because the artwork and statblock are to represent Vecna before Kas betrayed him (it was Kas who cut off his hand and cut out his eye).
But if that doesn't suit you as a lich he is able to reform his body - hands and eyes included - when he dies and unlike other lichs doesn't even need a phylactery to do so. So you can always just say the artwork is post reform.
There's also the Regenerate spell which he could easily gain access to given his vast network of cultists across the multiverse.

To attune to the magic items the eye and hand of Vecna, you need to remove your hand/eye. Shouldn’t this make you take damage. How much damage. Would it change depending on level to prevent low level kills or a easy attunement at high level
Damage is a representation of overall vitality and stamina - it's not always actual physical harm. Similarly loss of limb is not something fully covered in core rules and there's no rule or instance that says losing a physical piece, especially voluntarily, results in damage.
You can lose the relevant part and replace it with the magic item, no damage taken. It's roleplay flavour.

I assume Vecna can just make another eye. I mean, stats as present, he is a CR 26 undead with some of the highest damaging abilities in the game's official rules. He could have anyone working for him/ dominated by him. In Eve, he is coordinating thousands of cult cells across dozens of planets to gather secrets and reshape reality itself. I think that getting his hands on an Ersatz Eye wouldn't be a problem.
