Now I have many ways I was knocked out because the dms I played did not let my characters die. Though the dumbest is a bit confusing so I will try to explain.
was a player in one game but I was Dming a home brew one. So at one point I mentioned a ,merchant that was a sock puppet not knowing one of my players where afraid of puppets. So the merchant got killed right away. Later I mention that to the dm of the first one because he likes hearing about so when I mention the death of the npc. He made the final boss the daughter of the merchant and a white dragon. So I can know say I was killed by sock puppet.
side note one of the players left the game I was a player we found out the players character was flayed and crucified by the sock puppet.

I nearly killed a party member because of magical coral, bad rolls, and a monk rolling nat-ones on EVERY SINGLE REACTION! Did I mention that I was a half-orc barbarian with a glaive? I also nearly died because someone tried to toss me a healing potion and rolled a nat-1. It was a really easy throw, too.

There's always another secret --- A thing an OC says because I love references
During a session, I was running through an underground dirt corridor trying to avoid the tons of slime cubes that were scattered throughout the tunnels. I tried to jump over one at full speed, but my foot got caught on a mole-hole and I fell into a slime cube. I failed my saving throws and had a death-by-tripping-moment.
Had a wild magic autognome sorcerer who used magic gems and had a thick scottish accent.
In a fight with a void shark, I cast fireball - rotate to the orange gem, and fire it at them. 2 other players take their turn before I remember - "DM, I forgot to roll wild magic, should I roll now?". Yes, so I rolled a 1, then rolled the "fireball centred on self" result.
Junior (my character) looked down at the gem, which was still glowing, and said "Huh, that's not supposed to-", then died in a fiery explosion.

My DM homebrewed a spin a wheel game for numbers 1 through 20 casino where if you rolled a nat one you explode, guts and sinew everywhere

Luckily we had a cleric, so I got better!
