The rest of the Item is identical to a luck stone. My issue is getting it to add a bonus of 1 to initiative.

What am I missing?
Re-save the whole magic item. That will trigger the system to pick up the changes. Then be patient as it may take a while for the system to push those changes through to the character sheet.
