Here is a thing from Irish mythology I thought was cool and because my imagination might be a little too wild, it scared me silly last night.
The Fetch, or Co-Walker, or also a Waff or Reflex Man, is a thing from Irish lore. It is seen by someone, and it looks exactly like whoever was meant to see it. It is usually taken as an Omen of Death, meaning that whoever the Fetch appears to be, that person is fated to perish in the near future. The Fetch is harmless to all, except for its double. It is often seen by someone with second sight, which means that it might be a sort of fey creature. In German traditions it is called a doppleganger. At funerals, the Irish and Scottish wouldn't eat meat in case it brought them into contact with a Fetch.
I think these could be fun to use in Dnd to freak out your players. Maybe they are at a banquet, and suddenly a spectral figure of one of the guests appears, frightening everyone. Then that person begs the party to protect them from whatever will soon cause their death. Maybe a fetch is a fey creature who takes the form of someone that it knows will soon die, or maybe they hope they will die and try to kill them. Maybe a player comes across a duplicate of themselves who just stares at them, or maybe attacks them, and they realize that their death may be near. I would suggest that you give the party some information on the Fetch before the encounter, in order to make sure they understand its significance. Most people don't know what a Fetch is, so you should probably foreshadow its appearance by having them read about it or hear someone mention it beforehand.
I would say that a Fetch could use the shadow, doppelganger, or maybe the specter statblock, but maybe add this trait.
Omen of Death. One humanoid of the Fetch's choice has disadvantage on all death saving throws until the next morning. The Fetch can pick one target per day and regains the ability to do so at noon.
The weird timing of the Omen of Death feature is because seeing the Fetch in the morning is not necessarily a bad thing.

Me too. But I also read Colleen McCullough’s Masters of Rome series and they talk about them in there too (only talk about them cause Masters of Rome is not a fantasy series it’s historical fiction).
