*I did come up with one monster a while back that's rather evil... the Farseer!*
*The farseer is an extradimensional being that has some nasty abilities it can use in combat, one of which is its AC. Each time the farseer is attacked, instead of a set AC Score, the farseer rolls 4d4 + 4, and whatever it gets determines its AC against that specific attack. It rerolls its AC every times it is attacked afterwards, adding a sense of randomness to what hits and what doesn't.*
*The other ability it has that I think is truly vile is its Contact Volume of Zero. This ability allows all of its attacks to ignore nonmagical shields and armor, determining if the attack hits based on the target's Unarmored AC. This can be an absolutely vicious ability to have because that Warforged Forge Cleric with plate armor and a shield, which was previously sitting at an AC higher than 20, now has very little defense against the farseer's attacks.*
*Given how it also can go through walls and teleport to avoid an attack as a reaction, the farseer can be a ludicrously difficult fight (doubly so if it surprises the party by grabbing the squishiest player and just teleporting away with them). I only plan on using the farseer as a 'secret boss' of sorts, but I bet some DMs could do particularly nasty stuff by using all of its abilities in clever ways.*

Here is an evil plot. It is an adventure based on the Repairer of Reputations from "The King in Yellow".
Have the party come across some random man who asks for their aid. Lets name the guy Diggory just for fun. Diggory claims to be the king of a Nation but his evil brother has overthrown him. He needs the party to come with him and retrieve his magical crown and robes and he will be restored to his throne. They begin to sneak through a town and Diggory sends them off to kill his usurping brother. Now, they might kill him, or if they listen to their target for five minutes they will discover that both Diggory and his brother are normal citizens of a normal city with not a drop of royal blood in their veins. Diggory went mad after injuring his head in a horse accident and was influenced by another known madman into thinking he was actually a king. Diggory was exiled from the city for trying to murder his brother and is a very dangerous man. The party now has to go and capture Diggory who has boarded himself up in his old home. They meet another strange madman who is dressed up like a yellow robed monster that attacks them. They get inside and find that Diggory is wearing dirty old robes and a crown made out of painted cardboard. He seems harmless at first, but he carries a poisoned dagger in his sleeve and if that weapon is taken from him he will fight like an animal, biting and scratching anyone who tries to take away his crown. Once he has been apprehended, they might find a strange, yellow book that could be a plot device later on.
Throughout the story, Diggory will claim to have a vast supernatural power, but his brother will say that this is just part of his mad ravings. When they get into Diggory's house the place will be designed to look supernatural and dangerous but no magic will actually appear. They will see a strange monster dressed in yellow robes that will attack them sporadically, but it will really be the madman who influenced Diggory. There might also be a savage, hairy demon that turns out to be a crazed and starved cat or dog. There might be other "traps" that are really just the result of an ancient house in bad repair. Things like the floor giving away and falling chandeliers. I might put something in that turns out to be supernatural and out of place amidst a bunch of easily explained phenomena, but I just haven't figured that out yet.
This is just a small summary, I actually have a lot of smaller details planned out and might incorporate parts of M. Night Shyamalan's "The Village".

A hag lures the players into a mirror maze. The mirrors are magic and can copy the character’s attacks and spells. If a mirror is broken, then the character who broke it also takes damage. Once they escape the maze, they realize it was a portal to the feywild.
