They started the fight, but it's not that far in. It's going to end in a kidnapping, though. The sorceress of the party is running an emotional romance with one of the villagers. That villager's going to get snatched if the big guy wipes them out. The other villagers will of course save the party, but they can't get in the way of what's essentially their savior, even if he is a monster now. Anyway, she'll get dragged back to a massive kobold camp, the party'll find her a few levels later, and she'll pretty much be dead or on the verge of death, so I'm way ahead of you on that part.
No, my issue is what to do once they leave the village to pursue the monster. The whole map is pretty much a dense fog that invades your mind and pushes you towards insanity/suicide if you're not properly protected from it. It even has a feature where you roll a d4 for certain spells that decides if said spells work or go haywire. But I can't make that the whole trek, fun as it would be for me.
I need something for them to grow in. They're only level 3 now, which means they're all pretty soft. I need something that is cruel and rage-inducing, but in a way that helps them grow into something I can enjoy putting through the ringer. I already killed off the comedy relief, I figured that would be enough.
This campaign is R-rated, minus anything... physical, so don't be afraid to pitch me something bloody. I was thinking ankhegs, but they're a little too much right now. I don't want to slaughter them outright. Just maybe, I dunno, break them and play with the shattered remnants of their psyches.
So, I'm running a homebrew campaign I adopted from a dead thread. The premise is a ragtag bunch of adventurers making their way through a dense fog-laden valley in search of the son of their wealthy benefactor. So far, they have narrowly escaped drowning in a sea of mud, fought off opportunistic scavengers, weathered demented images of their closest family members and friends dying in front of them, endured the abduction and physical torture of one of their own (via PM duet), and are now fighting one of the BBEGs in a pocket dimension, surrounded by innocent villagers that are all being used as cattle for blood sacrifices.
I've already got an idea of what the villains will be, as well as a general idea of how the layout is going to work. My biggest issue is filling in the blanks.
Since you all sound like fellow sadists, keen on emotionallly torturing your players, I figured this was the place to be for ideas on making the trek from point A to point B as cruel and entertaining as possible.
Encounter ideas:
An ooze or shambling mound ambushes the players, trying to suck them into a pit of acid or quicksand
A cloaked ferryman offers passage across one of the valley’s swampy regions, provided the travelers are undead. If they’re living, he’ll offer to “cure” them
Swamp gas explosion
Cloud of supernatural bees or disease-bearing insects
A talking turtle, parrot, or alligator poses a riddle to our heroes
A rainstorm

You seem to have a swamp here so New Orleans style swing and jazz could fit. Maybe they come across a fun rest stop full of big band music, drinks, and happy dancers. The place has a natural charm to it and the party goers are eager to include the heroes in their revelry. You can set up events to teach them dancing and music and fill the place with interesting and fun characters to interact with a roleplay. The party might try to leave, but the dance hall won't let them. It's a sort of tantalizing trap. A safe sanctuary that won't let them leave, and maybe they need wisdom or charisma checks to work up the willpower to move on. When they do actually try to leave they see the place as it truly is without the glamour. Swamp drenched skeletons covered in bits of molding, rotted flesh shamble about the dance floor, devilish figures screech and clang on broken instruments, and bloated corpses riddled with squirming insects poor glasses full of filthy mud water and putrid poisons at the bar.

I’ve been DMing for a couple of my friends and my sister for a while now. The campaign is set in an Underdark dream realm that is impossible to leave created by a sapphire dragon being possessed by a nightmare spirit. The dragon’s soul was divided into eight gemstones which were hidden in the then-barren cavern, but each formed a specific and distinct environment based on the emotional state within that gem. The gems also subtly influence the forces of fate within the world, as it is a dream in essence. Actually, it’s a Domain of Dread but they don’t know yet…
Anyway, they’ve reached the Crosper region, and aside from accidentally breaking their iPhone Bird and forcing me to use SAM for TTS, they’re going to enter a ruined research facility for the corrupt corporation SREC. SREC used magical transmutation experiments to try and create an army of super soldiers to overwhelm the entire world (that they have access to), but created an immensely powerful insane hybrid humanoid called Rian. Rian, with access to all variety of monsters, data sheets, and tests of strength and cognition, went crazy and became the DM of the entire facility. The characters will play a simplified version of D&D within D&D, with Rian acting as a GLaDOS-like figure the whole time.

Amidais is startled a bit by Moonlight's sudden appearance, almost letting go of the Spider King before desperately grabbing on again to help pull him out of the demiplane.
"Yes... A- erm- a lot happened while you were gone." He gestures to the king stuck in spatial limbo. "C- could you lend me a hand?"

Yvonne and Elsa look up at the citadel, Elsa admiring its grand architecture, and Yvonne relieved to be somewhere vaguely safe again. "-Incredible... such beautifully carved marble! I'm--- still not used to architecture--- that isn't strictly functional." Elsa approaches the building, looking up at the ceiling in wonder.
Yvonne pauses for a moment, glancing over to the King while running her fingers across an etched rune on the wall.
"Drummer, I'm... I'm not sure what to do. We've only been back here for a few days, and... so much has gone wrong. I'm scared for Elsa, for the Guild..."
Meanwhile, Amidais is continuing to help his King. "Of course, sire... er, Sire? May I be so bold as to ask what our course of action is now? A- After getting you unstuck from the fabric of reality, of course."
The king staggers along with them, giving some strange hand gestures to some of the guards and priests, who immediately begin to open doors and get the chamber ready.
"It is... certainly not what we expected. I'm completely clueless as to how all this happened... we were gone for a while, something must have happened, something weird, duplication. Maybe one of the doctors ploys? The area also seems more prepared for war than I've ever seen... it worries me greatly." He nods at a high ranking priest, who begins to lead the trio into the libraries.
"To eliminate them." He says, almost casually, as he finally gets his head through, now only trapped by his final leg.The air almost ripples with every movement he makes to break free. "They're clearly a ploy made by this doctor that keeps hindering us. Even though we don't have the 01 they seem to want."
