Here’s a magic item I made for a god for the universe canvas.
The Monarch’s chalice (might also call it the forbidden chalice)
Artifact, requires attunement.
the monarch’s chalice is an ornate gold cup embedded with many rubies. The chalice can be filled with a nonmagical liquid when a creature holding the cup says the name of the liquid. The liquid never runs out, and anything from the chalice tastes amazing. An attuned creature can use the monarchs chalice as a decanter of endless water with a DC of 20. A creature attuned to the chalice can say the name of a magical liquid, such as a potion, and the monarch’s chalice will fill with that liquid. This liquid never runs out. A creature that drinks something made from the chalice must make a DC 10 wisdom saving throw, or a DC 15 wisdom saving throw if they drank from the chalice directly. On a failure, they become charmed by the chalice for a day. Every hour, the chalice casts suggestion (DC same as wisdom saving throw when drinking from the chalice) on every creature charmed by it to not only drink from the chalice, but to also commit an evil act. Every Time a creature drinks from the chalice, the wisdom saving throw DC increases by one. If a creature is charmed for more than 30 days, it becomes permanent.
The chalice has 20 charges. an attuned creature can cast the following spells with the charges while holding the chalice. Creatures effected by the spells must make a wisdom saving throw as if they drank something from the chalice. The chalice regains 5 charges at dawn.
2 charges: mass healing word, prayer of healing, purify food and drink.
3 charges: mass cure wounds, raise dead, vitriolic sphere, remove curse (can’t effect the chalice)
4 charges: greater restoration, heal, regenerate.
5 charges: resurrection, tsunami, heroes’ feast, mass heal.
10 charges: true resurrection, storm of vengeance, power word heal.
Forbidden drink
an attuned creature can speak a command word that causes the chalice to fill with golden liquid. A creature that drinks the golden liquid must make a DC 19 constitution saving throw at the start of every turn for a minute. On a failure, the creatures are poisoned and stunned until the end of their next turn. At the end of the minute, they gain a plus 4 to one stat of their choice and they gain a -4 to another stat of the DM’s choice. This ability can only be used once every 24 hours. There is only enough liquid in the chalice to effect one person.
blood of the monarch
an attuned creature can speak a command word to fill the chalice with the blood of it’s creator. Any creature that drinks from the chalice takes 2d6 necrotic damage. At the start of their turns, they can make a DC 20 charisma saving throw to stop drinking. On a failure, they continue drinking. A creature drinking from the cup takes Damage at the end of every turn. The damage starts at 2d6, and increases by 1d6 for every round the creature is drinking from the chalice. A creature that dies while drinking from the chalice becomes a hierarch, an undead sorcerer lich that uses their magically controlled family members as a phylactery. A hierarch created this way is a shadow sorcerer.

Okay, let's run with that. The players are trick-or-treaters (happy Halloween in June). The hag has been giving away little pastries covered in powdered sugar. The kids (not necessarily the players) eat them quickly to avoid making a mess of powdered sugar. Then, later on, the kids start having breathing problems. One of them suffocates right there... as hundreds of baby spiders pour out of his mouth and nose.
That's right, there were enchanted spider eggs mixed in with the powdered sugar. The cure is in the Hag's house, which is the classic funhouse dungeon of the elder days of DnD, complete with vampires, flesh golems, and animated pumpkins.
