I had moved out of range of counterspell at some point, so I was 70 feet away, you cast a spell with a range of 60 feet without declaring move, so that's assumed that you moved the minimum required amount, then I moved forward 30 feet and cast spirit guardians
so no, all evidence points towards me not being dead. and that's before I factor in the high probability of cat me fleeing from you for even more distance
with what are you attacking? you didn't stow the luck blade that you used to polymorph me, and to stow or draw for the first time a turn is a free action, but to do it twice, it's an action

Beginning at 5th level, you can attack twice, instead of once, whenever you take the Attack action on your turn.
let me just run through the round of me being a cat: I was polymorphed by you, that's your action, timer starts, a charging mammoth arrives and attacks you before you can take your one round of attacks uninterrupted, spirit guardians prevents you from closing the distance well enough, you (still) have to save twice to not fall prone, and you fail to reach me.
also, whenever you take the attack action. not you can attack once without using an action, but that is your action. you were holding a two handed weapon, that got transformed into a sword, you used some spells (requires a free hand), you didn't stow your newly acquired longsword, you pulled out the luck blade, and you would have to stow one of them to draw
I agree with iExist's logic on the attacking... you are doing a bit too much weapon swapping for it to be completely free... But I think that the spirit guardians actually should have gone away because there is no logical way that you can maintain concentration as a cat.

Two saving throws are required regardless of whether you killed him or not... So you can roll those.
