It really is. Only thing I would change is the 10th level feature, as to utilize it you have to change how your character plays. Unless you build your character smartly, you might have feats, magic weapons, or a fighting style
Now Tasha’s has the Martial Versatility feature, which allows you to change your fighting style whenever you gain an ASI, but it’s still a huge pivot. Also, the lack of viable bludgeoning and slashing ranged weapons means that most of the time those options won’t get chosen when the character gains Tools of the Trade
but, you might ask: “raccoon, why can’t the character just stick with melee?” Because then that wastes the feature, and and a fighter, if we’re thinking in terms of balance and optimization, they need everything they can get, unless you are intentionally choosing options for your character that fit more with the story at the loss of optimization

It really is. Only thing I would change is the 10th level feature, as to utilize it you have to change how your character plays. Unless you build your character smartly, you might have feats, magic weapons, or a fighting style
Now Tasha’s has the Martial Versatility feature, which allows you to change your fighting style whenever you gain an ASI, but it’s still a huge pivot. Also, the lack of viable bludgeoning and slashing ranged weapons means that most of the time those options won’t get chosen when the character gains Tools of the Trade
but, you might ask: “raccoon, why can’t the character just stick with melee?” Because then that wastes the feature, and and a fighter, if we’re thinking in terms of balance and optimization, they need everything they can get, unless you are intentionally choosing options for your character that fit more with the story at the loss of optimization

What about at some point in the higher levels letting them gain Expertise in Medicine? As a surgeon, it’s only logical.
Edit: Maybe at 10th level, replacing the long range targeting feature
