The description of this item says that when attuned the wielder gains 1 bonus hit point per character level. But when I attune it on a character the hit points on the sheet do not change. Is this a bug or am I doing something incorrectly?

I just had a look and can confirm that Berserker Axe does not give the extra hitpoints that it should.
Unfortunately this isn't something I can fix, as it requires a code change.
I have flagged this to staff.
