When trying to set up a prerequisite for a home brew feat, the only three proficiencies I can select as a prerequisite are;
1. Heavy armor
2. medium armor
3. light Armor
So when I want to create a feat to reward someone in combat for taking expertise in medicine by allowing them to hit vital organs and muscles Sherlock Holmes style.... I cannot do so :(

That is because the only official feats that require prerequisites are the armor master ones. As a general rule, feats do not have rerequisites and don't chain.
For your homebrew, you will have to rely on players only taking it if they should.
I think Houligan means proficiencies, as there are many other feats that have prerequisites, such as a minimum stat score or the ability to cast a spell. Unfortunately, despite appearances, the prerequisites portion of the feat homebrew only lets you pick from the patterns established by existing feats, and won't even let you put it in as a custom value. You can at least name the prerequisite after whatever you want, and then do something like require a 15 STR if that's what the player you want to give it to has. As long as the feat can still grant the benefit you want, you can fudge the prerequisite portion for now.
