That confuses a lot of people. They are intended to represent “greyed out” (left) and “lit up” (right).
I know this thread was from 2019 but I'm having trouble adding a homebrewed item I created to a character's inventory. I can see the item all ready to go in DndBeyond, but it's not showing up as an option to add to her equipment. Here is the item. https://www.dndbeyond.com/magic-items/6804982-springhold-greataxe
I have made the character proficient in greataxes so that shouldn't be the issue,

even if the character is Proficient with the item try unchecking the Proficient box/ all boxes in the Filter area and then search for the item. that's how I found my homebrew items
So, I know this forum is pretty old, but I figured I should add something for people stilling having this issue. I had the same problem, except I could add the item for the other PCs in my campaign. I figured out the that my player didn't have Homebrew content enabled. This could be found on the Home page of the Character Builder. Once I enabled it for my PC, the item started showing up in their inventory when I searched for it. Hope this helps!
What issue exactly; people in this thread have been overlooking various parts of the process such as not adding content to their collection, not having a subscription, or not enabling homebrew content. There isn't a single, overarching issue, so you'll need to be specific
Is the item in your homebrew collection? If not, that’s your issue. If it is, then check out General FAQ #s 5&6:

It is, but it still won't work
I just figured out that if it has a link you need to get rid of it before you try adding anything.
You shouldn’t have to.
